JOB PURPOSE:

Developing and implementing cardiac rehab treatment plans and providing cardiac education to patients and families. Able to perform patient assessment, exercise supervision and deliver emergency medical care utilizing established protocols for adolescent, adult and geriatric cardiac patients.

MINIMUM EDUCATION REQUIRED:

Master's degree in Exercise Science, Exercise Physiology, Human Performance, Adult Health & Fitness with concentration in cardiac health or a Bachelor's degree in Exercise Science, Exercise Physiology, Human Performance, Adult Health & Fitness with concentration in cardiac health required.

MINIMUM EXPERIENCE REQUIRED:

Minimum of one year's experience required.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:

BLS certified. ACLS to be successfully completed within 90 days of employment.

ADDITIONAL QUALIFICATIONS:

KEY RESPONSIBILITIES:

1.Performs patient assessments.

a. Assess patients initially according to protocol.

b. Provides follow up assessments based on signs and symptoms exhibited in rehab sessions and follow up medical care and testing.

c. Ensures use of appropriate tools-H&P, cath report, surgical report, discharge summary, GXT and significant other.

d. Interprets data accurately.

e. Reports the results of the assessment and follow ups accurately in patient chart and progress reports to physicians.

2.Develops treatment plans and exercise prescriptions.

a. Completes appropriate plan of care (HICFA for Medicare) following initial assessment and forwards top insurance company as required.

b. Prepares, adjusts and updates individual exercise prescriptions in accordance with rehab protocols based on GXT and physician recommendations.

c. Adjusts exercise prescriptions appropriately when usual procedures are not pertinent to patient.

d. Completes progress reports per protocol.

3.Supervises exercise classes per protocol.

a. Checks patients in and out of classes.

b. Completes attendance sheet: documents phone calls.

c. Interacts with patients during exercise providing motivation.

d. Displays appropriate judgment in supervising exercise by recognizing signs and symptoms of cardiac distress and taking appropriate action.

e. Displays appropriate judgment in supervising exercise for post surgery patients, deconditioned patients and/or patients with mobility or orthopedic conditions.

f. Displays competency in Phase II monitoring (includes telemetry, BP, HR, SO2, Glucose); enters patient information into the monitoring system and department databases.

g. Displays competency in Phase III monitoring (includes BP, SO2, Glucose); ensures information is entered on patient cards.

h. Transports patient to ED as necessary.

4.Educates patients and families appropriately based on diagnosis and need for lifestyle modification.

a. Displays ease and confidence in providing education as required in the outpatient setting based on patient need.

b. Provides information that is factual, current and consistent with medical information approved by the medical director and referring physician.

c. Assists inpatient educators at bedside and in group discharge classes according to pathways and established teaching plans as necessary.

d. Assists with Cardiac Awareness Class as needed.

5.Measures quality outcomes.

a. Assists with collection of data for outcome management by ensuring appropriate documentation in charts and compiling reports.

b. Assists with preparation of program recertification application as requested.

6.Participates in rotational training of new staff and student interns.

a. Demonstrates technical competency to new staff and student interns during the precepting period.

b. Ensures completion of competency checklist within 90 days for all orientees, or if extended, by the completion date of orientation.

c. Provides supportive, timely and constructive feedback during precepting.

d. Provides resources and development tools to foster a dynamic learning environment.

e. Completes or verifies evaluation for student interns as required by school.

f. Completes required checklists and documents in compliance with JCAHO and AACVPR standards.

g. Keeps other team members informed of learning needs for orientees or students to ensure a consistent learning environment.

7.Functions as a member of the Cardiac Rehab team.

a. Participates in equipment management and cleaning.

b. Participates in stocking of supplies and linens.

c. Notifies manager of supply needs.

d. Performs opening duties.

e. Performs closing duties.

f. Ensures phone messages are triaged appropriately.

g. Participates in orientation of new patients.

h. Assists with intake of new patients when requested.

i. Assists with entering patient charges when needed.

j. Assists with resolving patient account conflicts when requested.

k. Ensures appropriate filing of patient charts.

l. Participates in planning of social activities; attends at least one function per year.

m. Participates in one community activity per year: i.e. Heartwalk or Heart of Gold Games.

n. Attends minimum of 75% of staff meetings.

o. Serves as representative for department on committees or attends task force meetings as requested.

KNOWLEDGE, SKILLS, ABILITIES

  • Proficient in windows based computer software.

  • Excellent verbal and written communication skills.
