REPORTS TO: Manager-Community Based Services SUMMARY: Under direct, limited or consultative supervision, counsels clients requiring one or more forms of psychosocial interventions; interviews clients experiencing problems with individual, school and/or family adjustments; secures and evaluates applicable related information; refers clients to JF&CS services, community resources and other organizations. DUTIES AND RESPONSIBILITIES: * In a school setting, provides counseling to clients as appropriate to the position; prepares treatment plans; provides crisis intervention and emergency services as required. * Counsels clients individually, in families, or in other small groups. * Secures information on medical, psychological and social factors contributing to client's situation; evaluates these together with client's strengths and family support to formulate therapeutic approach or referral to other specialists. * Refers clients to JF&CS programs, community resources and other organizations as indicated; may determine or participate in determining client's eligibility for financial assistance. * Assesses client progress and modifies treatment plans accordingly; communicates with and counsels family members as appropriate. * Completes case record documentation and prepares reports; develops and/or reviews service plans and conducts follow up to determine quality and quantity of service provided client and status of client's case. Provides feedback to PQI Committee in summary form on results. * Accesses and records client and community resource information, manually or using personal computer. * Works as a team member with other professionals in delivery of care management services. * Plans and facilitates community outreach and education activities as appropriate to the position; may serve as a liaison or consultant to community organizations and schools. EDUCATION, SKILLS AND EXPERIENCE REQUIRED: * Masters degree in social work, counseling or psychology with experience directly related to the delivery of psychotherapy and/or social services. Special expertise in one or more areas may be required. * Provisional licensure as clinical social worker, psychologist or counselor is required. * Licensure as a clinical social worker, psychologist or counselor is preferred. * Interviewing, psychosocial assessment and diagnostic skills. * Ability to evaluate the progress of therapeutic plans and make individual modifications. * Experience in making classroom/community presentations. * Knowledge of community support services and resources. * Skill in preparing and maintaining accurate and timely case record documentation. * Skill in gathering data, compiling information and preparing reports. * Skill in use of office equipment, personal computers and related software applications.
