A full-service acute care facility serving the residents of Tracy for over 50 years, Sutter Tracy Community Hospital has a total of 79 inpatient beds. The hospital provides Basic Emergency Medical Services in a 9 bed E.D., soon to be expanded to 15 beds, inpatient and ambulatory surgical services, medical surgical, telemetry and coronary care, diagnostic imaging, laboratory and rehabilitation services, including physical, speech and occupational therapy. Areas of specialized care include a Hemodialysis, Wound Care and Continence Center, and a Family Birth Center.

Under the direct supervision of a licensed nurse, provides direct basic nursing care, including but not limited to: EKG's, simple dressings, CPR, patient valuables check, patient transports, vital signs, assist in equipment set up, ie., thoraklex, suture trays, and other trays, spinal immobilization, oxygen set-up, application of restraints, and post mortem care. Performs routine venipuncture to obtain blood specimens.

  • Training as an EMT, CNA, or MA current hospital/ED experience.

  • Ability to write, read, hear, and verbally communicate in English to the degree required to perform the job.

STCH provides a wide range of inpatient and outpatient services including: a 24-hour emergency department, inpatient and ambulatory surgical services, endoscopy lab, intensive care, diagnostic imaging, laboratory and rehabilitation services including physical, speech and occupational therapy. STCH also has specialized care for Hemodialysis, Family Birthing Center, Continence Center and a Pain Management Center. For more information about Sutter Tracy Community Hospital, please visit www.suttertracy.org The Wound Care Certified Nurse provides direct patient skin and wound management in both inpatient and outpatient settings. The Wound Care Certified Nurse plays an important role as a direct care provider, educator and resource for optimum patient outcomes in wound and skin care management and provides care for more complex patients under the supervision of a Certified Wound, Ostomy, & Continence Nurse (CWOCN). Skin and Wound Management includes the identification, assessment of patient/family needs, development of plan of care, direct implementation of care, prevention, quality monitoring, and continuing review/evaluation/modification as necessary of the plan of care (in accordance with established practice standards) for patients with alterations in skin/tissue integrity that includes but is not limited to pressure, vascular, arterial, and diabetic ulcerations, as well as skin conditions resulting from incontinence, surgical procedures and/or trauma. Skin and Wound Management is a specialized area that focuses on overall skin care and promotion of an optimal wound environment, including prevention, therapeutic interventions and rehabilitative interventions. The scope of practice of the Wound Care Certified Nurse is performed in accordance with legislation code, as determined by the California Board of Registered Nursing. MINIMUM QUALIFICATIONS: * Ability to read, write, hear and verbally communicate in English to the degree required to perform the job. * Current licensure as a Registered Nurse in the State of California. * Three (3) years recent acute care clinical experience related to the care of patients with wound/skin care problems or issues. * Current Wound Care Certification from an accredited school * Successful completion of STCH orientation/90 day provisional period. * Must be able to demonstrate communication, problem-solving, and critical thinking skills * Must be able to demonstrate age-related competencies for neonate through geriatrics * Must demonstrate the ability to provide education on wound/skin care and to adapt teaching techniques to skill level/teaching method preference of patient or care giver/family member learner. * Must demonstrate the ability to prioritize and manage multiple tasks * Basic computer skills, including PC and Windows applications * Basic Life Support Certification for Healthcare Providers * In depth knowledge of nursing process and performance improvement * In depth knowledge of current wound care standards of practice * See attached statement of Physical, Mental and Environmental Factors Form.
