Summary Benefits Supplemental Questions

  • Investigate TB cases and suspects and associate investigations which includes the identification and follow-up of all contacts e.g., skin test, x-ray, medical clinic appointments and ninety-day repeat TST's.

  • Responsible for all high risk screening activities along with other duties assigned by supervisor.

Grant funded.

Essential Duties and Responsibilities:
1. Contacts investigation interview, works up testing and referrals for TB cases and suspects
2.Responsible for all paper work related to contact investigation
3.Performs high risk screening activities
4.Locates, transports and medicates patients
5.Performs all other related duties involved in the operation of the Public Health Department as assigned or required

Minimum Requirements:

NOTE: YOU MUST FILL OUT THE WORK HISTORY and EDUCATION SECTIONS OF APPLICATION TO SHOW YOU HAVE YEARS OF EXPERIENCE/EDUCATION AS REQUIRED BY HIRING DEPARTMENT OR BE DISQUALIFIED.

TO APPLY, must have a Bachelor's degree or higher with major course work in Public Health, Natural Science or a related field.

TO APPLY, must have Two (2) full-time years of increasingly responsible disease counseling or prevention work experience.

TO APPLY, must have a valid driver license. Note: A Texas driver license is required to hold the position.

Must have the following knowledge and skills: communication both verbal and written; knowledge of medical terminology and forms, equipment, methods and techniques used to provide counseling services; principles and practices of public health education; pertinent federal, state, and local codes laws and regulations; communicable disease and how to protect themselves from disease process; computers and copiers and understanding of filing system.

Ability to work without constant supervision, type and enter data accurately and use a computer effectively maintaining a large changing database.

If hired, must provide proof of educational attainment

at New Hire Processing or during the promotional process.

Tarrant County will conduct background checks on new hires that will include a criminal background check related to convictions and deferred adjudications in the past seven years and may include credit reports, motor vehicle records, employment records and educational attainment. A conviction or deferred adjudication is not an automatic bar to employment. Each case is considered individually.

Follow up on non-medical case management services for patients seeking assistance from community agencies and assist with patient assistance and AIDS Drug Assistance Program (ADAP) applications for uninsured patients. Essential Duties and Responsibilities: * Coordinates referrals with social service agencies in and out of the Ryan White continuum of care. * Develops and maintains relationships with community partners to facilitate referrals. * Performs and documents initial patient interviews as assigned. * Documents all clinic and ADAP eligibility screens and services provided in required databases. * Receives and documents referrals from clinical providers. Develops a plan of care related to referrals before patient leaves the clinic if possible. Facilitates appointments. * Follows up and documents monthly "lost to care" activities. * Provides transportation support to eligible patients. Documents and tracks all transportation assistance tools. * Maintains clinic list of all ADAP eligible patients. Performs initial ADAP eligibility screening and recertification every six months thereafter. * Works closely with medical and non-medical case managers in care team to ensure well-coordinated medical care. * Attends ADAP trainings annually. * Performs all other duties as assigned. Minimum Requirements: The work schedule is: Monday, Wednesday, Thursday & Friday 8 am - 5 pm and Tuesday 10 am - 7 pm * High school diploma or GED. * One (1) year working in a social service or healthcare setting. * Experience with basic health education preferred. * Experience with HIV/AIDS preferred. * Bilingual in English and Spanish preferred. * Good organizational and time management skills. * Strong interpersonal and communication skills. * Detail oriented with the ability to follow-up with patients and services providers in a timely manner. * Possess outstanding customer service skills, especially with patients. * Basic computer and phone skills. * Ability to focus and complete tasks.
