Position/Job Title: Enrollment RN - Bilingual Job Number: 100200215 Employment Type: Fulltime Location: Los Angeles, CA 90001 Industry: Healthcare Salary: $45.00 Per hour Job Description: The individual will boost the PACE (Program of All-Inclusive Care for the Elderly) program, serving frail seniors and older adults in under-resourced communities across Los Angeles. As the Enrollment Registered Nurse, you are an integral part of the Outreach Team, which detects and assesses prospective participants for our PACE program. This role also functions as the connecting link between the IDT (Interdisciplinary Team) and the Outreach Team, guaranteeing an uncomplicated enrollment process for the prospective participants. Required Qualifications: - Graduated with an Associate degree or higher

• A minimum of 3 years experience in Geriatric Care or Senior Care

• Holds a valid CA driver's license, personal transportation, an undisputed driving record, auto insurance, and American Heart Association Basic Life Support (AHA BLS) and Registered Nurse (RN) in California (CA) certifications/licenses

• Fully proficient in speaking, reading, and writing in Spanish

• A minimum of 3 years of experience as an RN Preferred Qualifications:

• A minimum of 1-year experience with Enrollment or In-Home experiences Key Responsibilities: - Working in unison with the Outreach Specialist and IDT to guarantee successful enrollment

• Executing home visits to gauge the prospective participant's eligibility for the level of nursing home care

• Undertaking safety evaluations of the prospective participant homes

• Engaging in Enrollment Eligibility meetings to review Level of Care recommendations

• Performing Nursing Facility Level of Care (NFLOC) assessments and sending them to DHCS for review, updating/revise the assessment as requested by DHCS

• Accurately maintaining documentation and recording all evaluations in CRM software

• Engaging in all PACE Audits by DHCS and CMS - Supporting the Center Director to develop participant retention strategies

• Updating and enhancing knowledge and expertise in eligibility, enrollment, and program specifications for the PACE Program

• Encouraging business development and census growth by participating and assisting with outreach programs as needed. Schedule and Shift Details: - M-F, 8a-5p. Mostly in-field home appointments and occasional onsite work in Los Angeles clinic up to 5 days per month. Travel: - Travel to patient homes 95% of the time.
