POSITION TITLE: Enrollment Navigator (1 Year Contracted)

REPORTS TO: Licensed Vocational Nurse Manager

LOCATION: Ventura, County, CA

CLASSIFICATION: Exempt

SALARY: $45,000 with quarterly monetary incentives

PERTINENT INFORMATION:

  • Position to start immediately (January 2024)
  • Flexible schedule depending on outreach event.
  • Occasional evenings and weekend required (General schedule Monday - Friday, 9AM-5:30PM)
  • 2 openings of 8 hour shift

Enrollment Navigator/LVN

The Enrollment Navigator works in our Recuperative Care Program to provide outreach and on-site enrollment among other things listed below for the guests supported into gold coast health plan. Enrollment Navigators will have excellent bedside manners, are team players and have a “whatever it takes” attitude. The Enrollment Navigator will continue the torch for NHF recuperative program to provide quality and compassionate care and empower those we serve on site and within the community.

Responsibilities and Initiatives 

To help NHF meet its growth goals, the Enrollment Navigator will:

  •  Implement day-to-day program tasks, including rapport and relationship-building with individuals, providing community outreach and education to enroll/re-enroll individuals into Gold Coast Health Plan (GCHP), conducting community workshops and tabling events on the importance of GCHP insurance coverage and access to healthcare, providing local human services and medical resources information, communicating and maintaining partnerships with community members & partners, and other duties as assigned.
  • Monitor GCHP applications, provide updates on benefits and services, and stay engaged with participants to monitor their level of housing and health insecurity.
  • Enroll/re-enroll community members in GCHP and provide one year of case management through an initial 30 day follow up and then quarterly.
  • Share and provide resources on where and how to use GCHP benefits and monitor any re-certification dates that apply.
  • Stay up to date with Ventura City and County health and safety-net programs like CalFresh, SNAP-Ed, Medi-Cal and others, and how they can support community members and NHF programs.
  • Represent NHF at community meetings and build relationships with community members to increase NHF’s program participation.
  • Coordinate and/or assist with community events that promote programs, services, and NHF overall.
  • Maintain records of program deliverables, such as attendance sheets and event reach, and input data in evaluation dashboards.
  • Provide support to other team members, including but not limited to preparing workshop materials, assisting with program planning, etc.
  • Performing administrative tasks such as charting, guest summaries and answering phones. Project a positive professional image and adhere to organization dress code.
  • Flexibility to adapt to schedule changes to meet the needs of the facility.
  • Complete additional tasks/duties as required to meet the needs of the facility, not to impede primary responsibilities.

Requirements

  • At least 3 years of experience conducting community outreach and education as a certified or trained Community Health Worker or equivalent experience.
  • Knowledge of community resources, including public benefits, nonprofit organizations, and local businesses.
  • Experience working effectively with diverse communities.
  • Comfortable with public speaking and leading tabling and outreach activities.
  • Strong team player able to work effectively and efficiently with others toward common goals.
  • Fluent in English and Spanish, or other commonly used language in Ventura County.
  • Reliable transportation! This position will require you to visit multiple different sites over the course of a week and will need transportation.
  • Effective communication and social skills are essential to build and maintain relationships with guests and fellow employees.
  • Proven ability to meet challenging deadlines, prioritize and organize effectively.
  • Computer proficiency in MS Word, Excel, Outlook, web browser and proficient typing skills.

Physical Requirements

The duties as described will require frequent standing and walking, as well as extended sitting. The employee will frequently bend/stop, squat, reach above the shoulders, twist, and turn, kneel, and push/pull. The employee will frequently be required to lift/pull/push/move up to 50 pounds. Must utilize conversational speech for effective verbal communication. Noise level is high in some guest care areas. Must be able to hear equipment alarms, overhead pages, and direct verbal communication.

WHY IT MATTERS

Health begins long before that visit to the doctor. It begins where we live, learn, work and play.

On any given night, there are 46,000 Angelinos without a home. Of those, more than 29,000 have severe health issues requiring some level of care. Without a home, these individuals experience insurmountable barriers to health and healing.

In the 6 square miles that make up Historic South-Central Los Angeles, there are no gyms and there is only 0.4 acres of parking space for the 103,000 people who call the area home. Liquor stores outnumber grocery stores and there are no farmer’s markets. Without opportunities for safe outdoor play and places to purchase fresh fruits and vegetables, this community will continue to experience higher than average rates of obesity, diabetes, and cardiovascular disease.

High school can be a trying time, but for students who are pregnant, parenting, or at risk of becoming pregnant, the challenges are far greater. The impact of pregnancy on health is compounded in communities where poverty and a lack of maternal care are common.

If NHF accomplished its mission, everyone in Southern California would have the opportunity to experience health and would live in a community that supports a healthy lifestyle. That would include a home for every person, access to fresh, healthy foods, opportunities for safety, outdoor physical fitness, and so much more. We have already seen communities take great strides in tackling their negative social determinants, but really, we have only scratched the surface of these issues.
