General Summary:

Responsible for portaling and scanning paper documents into electronic medical records. Responsible for electronically processing patient health information. Ability to electronically import, fax and scan medical documents. Knowledge of medical terminology to correctly identify health information documents for electronic processing.

Principal Duties & Responsibilities:

  1. Portal patient records from outside health systems

  2. Obtain new patient records from referring provider

  3. Accurately scan medical records of new & established patients into EHR system

  4. Scan medical records obtained from other offices into EHR system

  5. Fax records to physician offices or Hospitals

  6. Answer phones

  7. Ability to identify patient medical record documents by name.

  8. Handle requests for release of patient medical information according to HIPAA rules and copy service contract.

  9. Other duties as assigned.

Knowledge, Skills & Abilities:


  • Ability to accurately identify medical record documentation by name for electronic filing.

  • Excellent customer relations and phone protocol

  • Ability to file alphabetically and numerically

  • Excellent organizational skills required.

  • Consistently arrives at work, in professional attire, on time and completes all tasks within established time frame.

  • Seeks appropriate tasks when primary tasks are completed and assists co-workers as needed.

  • Demonstrates adaptability to expanded roles.


  • HS diploma or GED


  • Previous experience in a medical office or in medical records.

The EMR Clerk is responsible for maintaining the patient's file. Insures that all patient documents, reports and transcribed medical progress notes are scanned and mapped in the appropriate chart. Scans incoming documents (labs, x-rays, transcribed reports) related to patient care and maps to appropriate area of the Electronic Health Record.
