Job Summary The Registered Dietitian/Inpatient Diabetic Educator provides assessment, planning, facilitation, and advocacy for optimal clinical and financial outcomes. In collaboration with the attending practitioner, the family and other members of the health care team, coordinates the individualized plan of care and communicates the plan to appropriate stakeholders.

The Registered Dietitian/Inpatient Diabetic Educator assists with appropriate use of resources both within the facility and external to the facility. Communication and collaboration may include coordinating admissions with referring facilities and providers, utilization review, discharge planning, transportation, and care coordination. The Registered Dietitian/Inpatient Diabetic Educator ensures that high quality care is provided as efficiently and cost effectively as possible.
