Educates patients and their caregivers on the cardiovascular disease process and procedures ordered to help determine treatment. Manages and coordinates care of the cardiology patient.

Coordinates testing and return to clinic to review outcomes of testing. Communicates effectively and collaborates with the multidisciplinary health care team to ensure seamless patient plan of care and follow-up and hand off to primary care provider. Other duties as assigned by the Nurse Manager.

Manages comprehensive nursing care for patients (within specialty area) whose needs range from uncomplicated to complex and rapidly changing. Based on holistic assessment, identifies relevant aspects of the situation and determines the appropriate course of action to move patient/family and staff towards the optimal health. Assesses subtle changes in patient status; anticipates problems before they occur and implements measures to minimize risk.

Organizes and prioritizes nursing care activities considering the needs of the patients and other staff members. Provides leadership to develop, analyze, integrate complex data, and update individualized care. Negotiates short/long term goals with the patient to develop and evaluate the individualized care when applicable.

Initiates/facilitates/collaborates with all members of the interdisciplinary team to improve care delivered to the patients, ensuring that appropriate referrals (interdisciplinary and community) are made and completed. Utilizes the nursing process to facilitate the patient's care using time and resources efficiently, and assumes accountability for re-evaluating care and its effectiveness. Coordinates care with the interdisciplinary members consistent with roles and responsibilities.

Designs, implements and evaluates a comprehensive, culturally sensitive, patient/family education plan. Uses professional judgment to appropriately delegate Makes age-appropriate interventions based on individual patient needs. Determines health needs of the clinics population.

Recommends behavioral changes, teaches skill, recommends medication adjustment s and reports to physicians. Demonstrates a caring, compassionate approach to patient/family, functioning as a patient/family advocate. Safely performs technical skills in area of practice.

Demonstrates practice based on knowledge of polices, procedures, nursing standards, and legal issues in areas of specialty. Coordinates and manages care by applying analytical reasoning, reflection, rational problem-solving skills, using evidence-based information and clinical judgment. Participates in patient care conferences communicating the individual patients progress toward optimal level of health . Participates in shared decision making and problem solving to promote effective collaboration with interdisciplinary team.

Serves as a clinical resource to other staff members and learners. Use established outcome measures to evaluate effectiveness of care. Promotes effective and efficient use of educational resources.

Collaborate with the Social Workers and other care providers to monitor, evaluate and trend patient responses to treatments. Manages resources to promote achievement of the best clinical outcomes that are targeted to the specific patient population. Assumes personal responsibility for quality patient care, environment and professional development.

Identifies own learning needs and seeks out resources to meet those needs. Attends national and regional conferences and networks with local organizations to maintain current expertise in specialty. Shares knowledge with others to improve patient care and to contribute to overall clinic functions.

Assist in the orientation of new staff members and students. Develops objectives for self and colleagues for professional development through continued evaluation of practice and by discussion with appropriate individuals. Demonstrates the insight and self-awareness necessary to recognize own impact on others and to change behavior accordingly.

Accepts constructive feedback and uses it to improve performance. Demonstrates effective use of computer-based information for decision support, continuous quality improvement and professional development. Actively and effectively assumes leadership role in clinic and in the community.

Demonstrates high standards of patient care and is a role model for others.

Delegates appropriately to others. Communicates effectively orally and in writing.

Promotes teamwork by facilitating positive group relationships. Demonstrates a spirit of service and cooperation in responding to needs of patients and co-workers, including ability to deal with stressful situations in a mature, positive manner. Contributes to clinic development by supporting and implementing activities that benefit clinic efficiency, productivity and quality and communicates practice standards to staff and others.

Demonstrates a professional commitment to patients and staff by developing clinic protocols, rules, and policies and assignments in response to patient and clinic needs. Follows through on meeting patient needs and requests, and addresses problems and systems issues in a timely manner. Demonstrates fiscal accountability for one's own practice while providing quality care.

Serves the surrounding community through presentations to groups of patients and families and/or by giving in-service training to health care professionals. Improvement/research activities designed to improve patient care, and/or organizational processes. Demonstrates an awareness of current trends and issues in area of specialty through discussion or application in practice.

Actively participates in clinic Performance Improvement. Demonstrates spirit of inquiry through ongoing and systematic evaluation of care given to patients. Participates in research studies available.

Pay will be commensurate with applicant's competencies as well as budget, equity, and market considerations. The ECU Brody School of Medicine is affiliated with Vidant Medical Center.
