TITLE: Early Intervention Service Coordinator

SUPERVISOR: Director of Early Intervention/Home Visiting and Contract Manager/Supervisor, Service Coordination

FUNCTION: Early Childhood/Family-Community Partnerships

SERVICE AREA: Stark County

JOB OBJECTIVE: To work in partnership with Stark County Board of Developmental Disabilities to provide Service Coordination for Early Intervention services for eligible children and families. Duties of the Service Coordinator includes: provide service coordination to assist the family to gain access to early intervention services; assurance of parents' rights; participation and cooperation within the Stark County Early Intervention Teams; coordination of the evaluation and/or assessment of the child and family; participation as an evaluation and assessment team member to determine eligibility, the child's need for services, and family assessment of the child and family's routine and needs; facilitation and participation in the development, implementation, and review of IFPS meetings; coordination, facilitation, documentation and monitoring the delivery of EI services as indicated on the IFSP; coordination and completion of the child outcomes summary; facilitation of the transition plan from EI to preschool services; and advocate for the child and family.

JOB REQUIREMENTS:

  • Minimum of a Bachelor's degree in Social Work, Education with focus on Early Childhood, or Nursing, or another field related to working with infants, toddlers, or children with disabilities.

  • Licensure Required: LSW, EIS/DS, RN, ECE-Professional Teaching License in Early Childhood Education, OT, PT, SLP, or licensed in vision impairment intervention.

  • At least two years' experience working with families.

  • At least two years' experience working with children ages birth to three.

  • Experience using child development evaluation and assessment tools

  • Knowledge of community resources.

  • Home visiting experience is preferred.

  • At least one-year previous service coordination experience is preferred.

  • Data entry experience.

  • Valid State of Ohio driver's license and proof of insurance.

  • Additional or alternative qualifications as deemed appropriate.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

  • Be the single point of program contact for carrying out the activities of service coordination, including assisting the parent in gaining access to early intervention services.

  • Assure parents have been informed of their rights. Assure Parents' Rights are communicated verbally and in writing, and consent is obtained.

  • Participate as a member of Stark County's Early Intervention Team

  • Assure that developmental screenings have been completed. If a delay is suspected, or the parent requests, then a referral for a developmental evaluation must be made with the consent of the parent.

  • Assure the coordination of the child's evaluation, child assessment, and family assessment within a 45-day timeline.

  • Assure the development and implementation of the initial, annual and periodic reviews of the Individualized Family Service Plan (IFSP) per mandated timelines.

  • Assure the coordination, facilitation, documentation, and monitoring the services delivery of early intervention services to ensure services start within 30 days of a valid IFSP.

  • Assure the completion of the child outcome summary

  • Facilitate development of family outcomes.

  • Assist parents in identifying, establishing, and communicating with a medical/health home.

  • Identify specialized services and other providers.

  • Provide choices to families by identifying all service provider options.

  • Inform families of the availability of advocacy services.

  • Coordinate with medical and health providers.

  • Coordinate transition to other programs and services.

  • Assure and facilitate the development of a transition plan from early intervention to special education pre-school, if appropriate, or other services.

  • Demonstrate knowledge and understanding about Part C of IDEA and children who are eligible, the nature and scope of available services, and the system of payments for services in Ohio.

  • Responsible for maintaining appropriate documentation for the client record (hard copy and electronic data systems) according to ODH and/or DODD local rules/policies.

  • Communicates and cooperates effectively, professionally, and respectfully with families/schools/professionals, and all others involved in the early intervention system.

  • Contributes to team effort by integrating individual responsibility with other team member responsibilities.

  • Demonstrates commitment to continuous learning and self-improvement by reading and attending such meetings, conventions, and conferences as are necessary to keep abreast of the latest early intervention and early childhood developments and trends.

  • Complies with the policies and regulations of the county Superintendent of Schools, and the mandated state Early Intervention rules.

  • Performs such other duties as may be assigned by the Director, Supervisor/Contract Manager of Early Intervention and/or the county superintendent of schools or his/her designee.

  • Obtains and maintains State of Ohio Service Coordinator credentials according to state early intervention mandate.

  • Evening and weekend hours may be required.
