Early Childhood Screening Nurse

  • Possesses certification as a Registered Nurse in Minnesota.

  • Strong public and human relations skills.

  • An ability to work well with students, parents and screening staff.

  • An understanding of the procedures involved in doing Early Childhood Screenings.

  • Ability and willingness to commit to the school district's mission, beliefs, and strategic plan.

REPORTS TO: Early Learning Coordinator

JOB GOAL: To strengthen the Early Childhood Screening process by providing health/medical expertise and by serving as the medical personnel member required by state statute to be present every time early childhood screening is being done.


  • Checks, records and graphs the height and weight of each child who attends the district early childhood screenings (ECS).

  • Screens hearing and vision and records the results for each child who attends ECS.

  • Reviews the health history, immunization records, insurance coverage information and family factors forms turned in by the parents for each child who attends ECS.

  • Shares and discusses any health concerns with parents and provides health education information for them as requested.

  • Writes referrals for children who do not pass the vision and/or hearing screening. Shares this referral information with parents and does follow up phone calls on previous referrals made.

  • Completes the health/immunization portion of the overall screening summary and makes notations about any concerns. (This information is then passed on to district kindergarten personnel, nurses and building health aides for future reference and planning when children enter kindergarten.)

  • Maintains records of health assessments parameters.

  • Assists with State reporting.

  • All other duties as assigned by ECFE Coordinator (or designee).

TERMS OF EMPLOYMENT: As per employment agreement.

EVALUATION: Performance of this job will be evaluated annually.

Also provide health and wellness education for individual patients and groups as requested. Also provides comprehensive home care services to patients and families with chronic disease and disabilities, as assigned. Job Duties: * Provides primary nursing services in a clinic setting for SMSC Community members, employees, and dependents. * Also provides home care for SMSC Community members and dependents to include primary nursing services, personal care, and related care as needed. * Works closely with the multi-disciplinary team to include Medical Doctors, Chiropractors, Physician's Assistants, Nurse Practitioners, Physical Therapists, Occupational Therapists, and other health care professionals. * May provide initial assessment and medical treatment for SMSC employees who are injured at work and secure a urine specimen for drug and alcohol testing. * Administer health screenings for employees as part of a health risk assessment to include biometric testing, health questionnaire, and recommendations to reduce or eliminate health risks. * Give immunizations to patients to include but not limited to influenza and hepatitis vaccines. * Document patient information gathered during the assessment, biometric testing, observation, care provided, patient progress, and any relevant medical information for each patient within the confidential patient record. * Provides health education opportunities for staff on various health-related topics as requested. * Complies with HIPAA rules and maintains all patient information confidentially and securely. Job Requirements: * Successful completion of a Practical Nursing program from an accredited secondary school. * Must be currently licensed as a Licensed Practical Nurse by and in good standing with the Minnesota Board of Nursing. * Good computer skills to include MS Word, Excel, and Outlook, as well as other health care or business related software as needed. * Excellent verbal and written communication skills. * Must hold and maintain Cardio Pulmonary Resuscitation (CPR) certification. If not CPR certified upon hire, must obtain certification within 90 days of employment. * Valid driver's license required with good driving record. This position will be require flexability to work between 7am - 7pm. We offer urgent care on the weekends, this position will be on a required rotating schedule for weekends and holidays.
