About the Role:

The Case Manager is responsible for providing Rapid Re-Housing services to households meeting the Department of Housing & Urban Development’s (HUD) definition of literally homeless, including those fleeing/attempting to flee domestic violence.

The Case Manager is to implement services with a trauma-informed, strengths-based and client-centered approach. The Case Manager will work closely with the Senior Case Manager of Rapid Re-Housing to accomplish program goals and priorities in accordance with agency philosophy and values and HUD grant requirements.

How you will contribute:

  • Develop Housing First focused service plans, individualized to the client’s unique housing needs
  • Perform assessment of client’s physical, mental health and social needs, life skills needs, employment history, educational needs, safety, strengths and other relevant areas to determine, in conjunction with the client, what supports are needed to develop and achieve housing goals
  • Provide crisis intervention and develop individualized safety plans as needed; assist victims of domestic violence in filing for protective orders and completing affidavits; accompany clients to court proceedings and various appointments (i.e., attorney meetings, protective order hearings, custody hearings, and housing interviews)
  • Coordinate services amongst mental health service workers, service providers, advocates, the legal system and other involved parties
  • Provide housing search assistance to clients; support clients in developing and maintaining a budget
  • Communicate and establish working relationships with landlords to negotiate lease terms and advocate on behalf of clients
  • Disseminate information to related to tenant rights and responsibilities, housing discrimination, communicating with landlords, basic unit maintenance, lease compliance, and housing search and location techniques
  • Coordinate the provision of rental assistance, track all payments provided
  • Maintain current knowledge of issues impacting homelessness and domestic violence.
  • Work with the Senior Case Manager to ensure that services are implemented in accordance with agency philosophy, best practices, and all applicable federal, state and local regulations
  • Work with the Senior Case Manager to ensure compliance with all HUD grant requirements, including the completion of environmental reviews, utilizing HUD funds for costs eligible under the Interim Rule, entering accurate client data into HMIS, VADATA etc.
  • Document all contacts with clients in client file HMIS and VADATA as required; maintain complete client records in accordance with agency and best practice standards
  • Participate in staff meetings and trainings as directed by supervisor.
  • Proficiently utilize Microsoft products as a part of day-to-day work requirements.

Requirements

Required:

  • A bachelor's degree in human services/ related field or commensurate experience
  • 1+ years of case management/ direct services /non-profit/ diverse population experience
  • Strong written and oral communication skills
  • Ability to prioritize competing priorities and make sound judgements
  • Ability to complete tasks while navigating frequent interruptions
  • Ability to deliver creative, resourceful solutions to unique challenges
  • Strong organizational and time management skills
  • Valid Driver’s License and reliable transportation
  • Ability to Drive a minivan
  • Flexibility to work outside of normal business hours when needed (7am-7pm)

Preferred:

  • Experience with providing services to clients experiencing homelessness, victims of domestic violence/ sexual violence, mental health or substance abuse issues
  • Foreign Language fluency in a language other than English (ex. Spanish, Farsi, Arabic or Amharic)

Physical Requirements: 

  • Annual TB Test is required 
  • Ability to sit or stand for long periods 
  • Ability to lift items weighing 10-20 pounds
