Per Diem Position with a flexible daily schedule.
Driver/Porter Position Summary:
Provide transportation to residents for approved appointments. Provide regular maintenance of assigned vehicle (including cleaning and oil changes). Organize and manage the daily transportation schedule.
Essential Duties and Responsibilities:
A. Coordination/ Communication
Communicate daily with team regarding who is being transported to and from appointments. Propose schedule adjustments.
Communicate with the assisted living, wellness, nurse, and Center for Health Rehab teams on scheduling appointments.
Provide safe and efficient transportation to and from appointments for residents.
Participate as a member of the multidisciplinary team in the development, implementation and evaluation of the assistance level needed while out in the community. Confer formally and informally with other team members in coordinating the transportation and functional level of the resident.
Monitor resident transportation assistance needs and modify vehicle used or assistance provided. Provide/recommend adaptive devices for transport.
Review the quality and appropriateness of the total transportation services and schedule for effectiveness and efficiency.
Perform general cleaning and maintenance functions, including car washes, internal cleaning and timely oil changes. Communicate any other maintenance that is needed. Inspect vehicles at the onset of each shift for cleanliness and safety.
B. Safety/ Health/ Confidentiality
Maintain a safe and clean work environment and adhere to facility and State/municipal safety standards.
Comply with all Infection Control, Universal Precautions and OSHA standards for the healthcare professional.
Adhere to established confidentiality standards as required by Burcham Hills Retirement Community concerning community and resident information.
Assume responsibility for ongoing continuing education and professional development as required.
Education and Experience:
Possesses a current and valid drivers license with an excellent driving record. Chauffers license preferred.
Prefer CNA certification and experience in an acute care, long-term care, assisted living community or other related health care setting.
