Responsibilities
1.Transports our clients safely to their destination.
2.Maintain all logs and reporting requirements (vehicle checks and tracking mileage.)
3.Assists with program delivery needs.
4.Responsible for bringing vehicle in for regularly scheduled maintenance appointments and keeping the vehicle clean.
5.Will be responsible for transporting client?s medications and cell phones between routes.
6.Observes LHBS safety and security procedures; determines appropriate action beyond guidelines and reports potentially unsafe conditions to Transportation Coordinator. Uses equipment and materials properly.
7.May require limited overtime, on limited occasions.
8.All other related duties as assigned.

Transports our clientele from our treatment programs to residential placements, public transportation hubs and related health care appointments.

  • Qualifications

High school diploma or general education degree (GED).

Current and valid driver's License, and a proven record of safe driving.

Must demonstrate compassion for populations served in LHBS and have a willingness to work in a multicultural setting and share a customer service orientation.

  • Physical Requirements

While performing the duties of this job, the employee is regularly required to sit for long periods of time

while transporting clients. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include distance vision. While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. Since the position sometimes requires transporting multiple individuals, the noise level may be moderate. The employee may need to become involved in the management of the behavior of passengers.

The Staff Nurse will be responsible for: * Providing care for patients in Primary Care * Working collaboratively to provide excellent care to patients and families in the safest environment. To qualify you must have: Education: * Graduate of an approved school of nursing with current registration in Massachusetts. * Bachelor of Science Degree in Nursing is required. Experience: * Recent Ambulatory Care experience required (surgical/procedural excluded). * Experience with telephone triage of adult patients in primary care, urgent care or the ED required. * Three years of Primary Care experience preferred. * Bilingual: Spanish/English speaking preferred. * Previous experience with medical residents preferred. Desire to work with nursing students and medical trainees is essential. Other: * Membership in professional organizations is recommended and certification in a specialty area is encouraged. * Able to work weekend rotation per the MNA contract. SKILLS AND ABILITIES: Must meet all skills and abilities requirements on the BWH RN job description and additionally: * Clinical knowledge and expertise includes basic health teaching, family planning education, knowledge of advanced care directives, care for common illnesses. * Experience with adult populations required. * Demonstrated knowledge of hospital-based and community-based care. * Community-based care experience desired. * Excellent communication skills, professionalism and organization required, demonstrates patience, compassion. * Excellent analytical, judgmental, and interpersonal skills and demonstrated ability to work effectively with patients, families and co-workers * Certified Diabetic Educator preferred /Interest in becoming a CDE * Motivational interviewing/Health Coach Certificate preferred /willing to achieve within two years of hiring. * Technical dexterity, in addition to Epic, must possess a demonstrated ability to work with a variety of information systems and databases, including but not limited to: MS, Access, Excel - required * Ability to be flexible and maintain equanimity in a fast paced, rapidly changing clinical setting – required. * Must have the ability to function at the highest level professionally and clinically and further possess excellent interpersonal skills.
