Job Summary

This position is responsible for the management and successful operation of Long Term and Community Integration Services (LTCIS). Provide supervision, training, and guidance to assigned staff. Oversee the development and implementation of Individual Service Plans (ISP) and support to assigned participants. Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and CARF as well as other funding sources and company policy and procedure.

Primary Duties

  • Supervise assigned employees to ensure their duties and responsibilities are completed. Hire, train, and evaluate assigned employees. May resolve work problems or assist employees in solving work problems.

  • Manage the daily operations of the LTCIS.

  • Ensure that the needs of the participants are being met and monitor services being provided by assigned personnel. Ensure that employees provide comprehensive and appropriate services to the participants receiving services in conjunction with contract specifications and supervisory instructions.

  • Maintain knowledge of and ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and CARF as well as other funding sources and company policy/procedure.

  • Ensure the employees provide accurate and timely performance reports, invoices, etc. Ensure that employees provide comprehensive reports including accurate, complete, and timely reports and statistical reports on the progress of assigned participants.

Additional Responsibilities

  • Assure that employee files are being maintained to meet the standards for all regulatory agencies.

  • Attend county Behavior Management and/or Human Rights Committee to present behavior intervention programs for participants, as needed and ensure crisis intervention and behavior intervention services for all participants as needed.

  • Coordinate services with Community Services Board Support Coordinators to facilitate total program services

  • Complete and ensure employees complete in an accurate and timely manner all evaluation/assessment reports, progress reports, daily notes, incident reports, attendance, and other supporting documentation.

  • Meet corporate goals for LTCIS revenue through filling programs with appropriate referrals.

  • Conduct regular meetings with LTCIS personnel you supervise.

  • Attend relevant meetings/committee functions internally and in the community to include peer agencies and advocacy groups.

  • Meet with representatives of other community agencies, families, and significant others for the purpose of participant planning and review.

  • Coordinate and participate in Interdisciplinary Team process and other meetings pertaining to participant's goals and objectives, as needed and oversee the development, writing, and implementation of Individual Program Plans and the maintenance of participant files to meet Medicaid, Licensing, and CARF standards.

  • Perform other responsibilities as assigned.

Qualifications: Education, Experience, and Certification(s)

  • Required: Bachelor's degree in a human service field including, but not limited to, sociology, social work, special education, rehabilitation engineering, counseling, or psychology.

  • Preferred: Master's degree in special education, vocational rehabilitation, or a human service field .

  • Required: Five (5) years' experience in the human service field.

  • Required: Three (3) years' supervisory experience.

  • Preferred: Minimum one year (1) Supported Employment or Vocational Rehabilitation experience with people with disabilities.

  • Must pass State of Virginia Department of Behavioral Health and Developmental Services (DBHDS) fingerprinting and criminal background check.

  • Must pass the ODD (Orientation to Developmental Disabilities) assessment and competencies.

  • Employees who drive individuals in the community or on behalf of the company must possess a current and valid driver's license. Eligible drivers must have a good motor vehicle record (MVR). Driving requirements vary by site. If eligible to drive, must pass the online driving training within 15 days of hire.

Knowledge, Skills, and Abilities

  • Physical ability to support participants with personal care (feeding, toileting, transferring, etc.).

  • Ability to relate well to people at all levels of an organization.

  • Proficiency in Microsoft Office skills, and the ability to become familiar with corporate specific programs and software, such as Microsoft Teams or ZOOM.

  • Ability to solve practical problems and adapt to new information and guidance quickly.

  • Excellent verbal and written communication skills.

  • Detail-oriented and able to carry out work with the highest levels of accuracy.

  • Ability to work independently and as part of a team.

  • Knowledge of behavior intervention principles.

  • Knowledge of community resources.
