The Discharge Planner assists the treatment team in understanding the barriers to discharge planning. The Discharge Planner is responsible for having a thorough understanding of the patient's treatment through communication with the treatment team.
The Discharge Planner advocates for the patient's access to services during treatment team meetings and through individual physician contact. The Discharge Planner will carry a caseload, will meet with patients to discuss discharge plans, be responsible for completing referrals, and coordinating with outpatient providers to arrange outpatient care. On an as needed basis, the Discharge Planner works with the Utilization Review department on certification and recertification of insurance benefits.
1.Bachelor's degree from an accredited college or university in social work, mental health, nursing, or related degree and two (2) years of behavioral health experience required, Master's Preferred.
2.A minimum of two (2) years' experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management.
3.Valid CPR upon hire.
