Cypress Creek Hospital is a free-standing 128-bed, acute care psychiatric hospital located in north Houston, TX, and minutes south of The Woodlands. Cypress Creek Hospital features individual units for adolescents and adults and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. On average, over 10,000 patients receive care from our compassionate health care team each year at Cypress Creek Hospital. We currently have opportunities for full-time Discharge Planner.

The Discharge Planner is a key member of the patient's treatment team who assists patients and their families with their discharge needs.

Job Duties/Responsibilities:

• Provide timely and appropriate discharge plans for patients.

• Maintain documentation of discharge services provided.

• Keep psychiatrist and treatment team informed of discharge plans.

• Attend weekly physician meetings to give/gain information regarding discharge plans of patients.

• Attend team and therapy meetings and provide consultation to treatment team.

• Interface with the Utilization Management team on a regular basis to determine patient discharge needs within managed care counseling.

• Attend mandatory department meetings.

• Other duties as assigned.

Qualifications

Discharge Planner Job Requirements:

• High School or GED diploma, bachelor's degree in psychology or related field preferred.

• Two (2) years related experience in a mental health role or a combination of education and experience totaling a minimum of two (2) years.

• Experience dealing with community resources desired.
