Discharge Planner / MSW - PRN Days

Coordinates care of an assigned caseload of patients through collaboration with physicians, nursing staff and other health professionals to facilitate efficient utilization of clinical resources, optimal clinical and continuing care as well as financial outcomes. Intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high risk populations. This role will receive referrals for individuals from interdisciplinary team members. This role will provide services in all patient areas as needed. The social worker collaborates with all members of the healthcare team. May supervise LBSW's or LGSW's and function as a preceptor for social work students.

• Masters Degree in Social Work required.

• Licensed in Alabama as an LGSW or LCSW or must obtain license within 12 months in position.

• Three (3) years of hospital experience preferred.

• Knowledge of case management.

• Experience with skilled nursing facility, home health, hospice, child, adult, elder abuser and neglect. and rehabilitation preferred.

• Case Manager (CCM or ACM) certification preferred.

• Able to maintain a professional and customer centric composure in difficult or challenging situations.

• Ability to work independently with minimal supervision.

• Must be able to function in high stress environment subject to frequent interruptions and changing priorities.

• Ability to prioritize workload to achieve maximum efficiency.

• Ability to effectively communicate with people from various backgrounds throughout the organization.

• Ability to evaluate and assess difficulties in age specific psycho social functioning.

• Ability to Communicate effectively, both orally and in writing.

• Must possess basic computer skills. Must know medical terminology.

• Excellent organization and time management skills.

• Licensed Master Social Worker LMSW or LICSW within 12 months in position
