In partnership with the Medical Director of Post-Acute Care and service line leaders, collaborate with post-acute facility leaders in planning, assessing, creating and enforcing accountability to drive patient quality performance indicators. Establish standard and systematic approaches of communication to and about the value of Novant Health's post-acute network. The Director Post-Acute Care serves as the Novant Health administrative representative to all skilled nursing facilities and home health care across the Novant Health footprint.
Education: Bachelor's Degree required.
Master's Degree preferred. Clinical professional degree in nursing or other clinical discipline preferred.
Experience: Minimum five years in management/leadership experience, preferably in skilled nursing home facility or nursing.
Minimum ten years of clinical experience. Previous experience as a clinical leader preferably in post-acute care. Demonstrated ability to build strong, collaborative relationship and partnerships with internal leaders and external post-acute network leaders. Additional experience preferred in value based models/episode of payment models.
Additional Skills Required: Responsible for engaging and collaborating with post-acute care providers, partners, physicians and care management.
Foster relationships which foster expectations and accountability. Have a foundation of analytical, problem solving, performance monitoring, strategic intelligence, time management, project management and organizational skills. Effective written and verbal communication.
Ability to prioritize and manage multiple operational or strategic initiatives simultaneously. Monitor and communicate performance metrics. Knowledge of episode of care payment models, governing standards, policies and procedures; skills in interviewing and selection process; understanding of budgeting, staffing, payroll and purchasing processes.
Ability to lead and develop a team which includes: team building, providing feedback, staff development and progressive discipline processes. Willingness to work varied hours and travel to multiple offices 25% of the time. Ability to manage staff in multiple cities and locations. Ability to be a self- starter, team player, works in matrix environment, contribute to a positive working environment and demonstrate initiative, ownership and accountability.
