The Director of Palliative Care Services position is located in Indianapolis, IN.
The Director will provide leadership for all Palliative Care Services throughout Community Health Network and have an administrative dyad with the Palliative Care Services Medical Director. Areas of responsibility include:
Overseeing operations in Acute Care and Ambulatory Palliative Care teams across 6 Community Hospitals, 5 clinics located in 3 Community Cancer Centers, 2 Advanced Heart Failure Clinics, and Provider Home and Facility Consults within the 3 Indianapolis regions. This includes strategic planning and budget management.
Developing strategies and implementing systems to ensure:
Excellent patient and family experiences which continue to improve,
Efficient and effective use of resources that meet national baseline benchmarks,
Integration of services and programs for efficient patient navigation through an integrated continuum of care,
Quality indicators which meet national baseline targets, and
Continued compliance with legal, regulatory, and clinical policies and procedures.
Leading teams including all Palliative Care Nurse Practitioners, Nursing staff, Social Workers and a Practice Manager, with clinical oversight conducted by Palliative Care Medical Director.
Speaking for and representing our services internally and externally in areas of growth, innovation, and philanthropic support.
Minimum of 5 years' experience in a health care related business, with experience in program development.
Experience in Palliative Care preferred. Must be certified within two years of start date.
At least 2 years' experience working as part of an interdisciplinary team.
At least 2 years' management and leadership experience preferred in a health care setting.
Bachelor's degree in health related field required; MBA with a specialization in health related field preferred.
Strong relationship development and communications skills.
Ability to prioritize and work independently.
Demonstrated competence in Microsoft Office, Excel and Web navigation.
Proficient organizational skills for office management.
