Shipley Living is recruiting for Director of Social Services. This role plans, organizes, develops and directs the social service programs to ensure that the medical related emotional and social needs of the residents are met and maintained on an individual basis

Here are a few of the daily responsibilities:

• Assesses and identifies medical-related social and emotional needs of the residents. Develops and implements social care plans with the involvement of the resident, his/her family, physician, and other nursing care staff.

• Consults with physicians and healthcare team members in dealing with psychosocial factors relating to residents' illnesses, treatments, and discharge plans.

• Provides discharge planning, including referrals to community and family social services.

• Complete all MDS and related assessments within a timely manner.

• Assists residents with financial and legal matters; provides or arranges for counseling.

Here are a few of the qualifications we need you to have:

• Bachelor's Degree in Social Work from four-year college or university

• Minimum of two years related supervisor experience and/or training ACSW registration.
