Director Of Social Services

Haven Homes, a 5-star rated senior care campus, is hiring a Full-Time Director of Social Services to join our team at our beautiful, brand-new building. The Director of Social Services will advocate for our residents, families, and staff and provide overall leadership and management. Our ideal candidate thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills. Having a license in social work is preferred but not required.

We moved into our premier 149-unit independent living, assisted living, TCU and long-term care senior health & living campus in December 2020. We are located a mere 12 miles from Ridgedale Mall, just off Hwy 12 in Maple Plain.

This is a Full-Time, benefit eligible position.

Director of Social Services responsibilities:

• Provide guidance and direction to the social services department through policy and procedure creation and monitoring to ensure the Cassia mission is being realized.

• Coordinate caseloads, complete social history, care plans, MDS, and discharge plans for new residents.

• Engage with community partnerships.

• Maintain and supervise the budget.

• Participate in resident care conferences involving the planning and implementation for medically related social services, mental health services, and emotional support services.

• Oversee and complete referrals for resident mental health services and coordinate consultative services with providers.

• Complete social services admission evaluation, as well as discharge, care plans for residents.

• Maintain necessary documentation in resident's medical records.

• Perform other duties as assigned.

Director of Social Services skills and requirements:

• License in Social Work preferred but not required.

• LTC experience preferred.

• Previous supervisory experience preferred.

• Prior experience in the admission process, discharge planning, or management preferred.

• Strong knowledge of local, state, and federal guidelines related to long-term care and resident rights.

• Strong leadership, written and verbal communication skills.

• Good communication and the ability to multi-task.
