Job Summary:

The Director of Risk Management and Quality Improvement, RN will be responsible for leading and overseeing the clinical risk management program at CCCN, promoting sound clinical management principles and strategies that identify risks in the clinical setting leading to the coordination and the development of action plan(s) to reduce opportunities for harm to patients, visitors and staff. This position will also be responsible for development and implementation of network-wide education for risk management issues, risk control approaches and compliance with external accrediting agencies standards and state and federal regulations. This role is ensuring the highest standards of patient care, compliance, and operational excellence.

Qualifications

Qualifications:

  • Bachelor's degree in nursing required; Master's degree preferred.

  • Data management experience, statistics, process improvement methodology and PC applications required.

  • Minimum 2 years skilled nursing facility experience required.

  • Minimum 2 years risk management experience required.

  • A current RN required; Certification in Risk Management preferred.

  • Ability to travel to facilities and provide a hands-on teaching approach.

  • Ability to manage all forms of communications.

Essential Functions:

Risk management:

  • Develop, implement, and manage a Risk Management Program that identifies, analyzes, and mediates potential risks to resident and/or employee population for the Company.

  • Conducts root cause analysis of incident(s) and develop strategies to prevent recurrence

  • Ensure compliance with federal state and local regulations related to risk management

  • Reports data to the required agencies and to the State Department of Health regarding mandated reporting of Preventable Adverse Events.
  • Documents, analyzes and reports risk metrics, incident data and other information as required by regulatory and accrediting agencies.

  • Receives and responds to risk-related patient complaints and incident reports. Assists with timely resolution including development of corrective action plans.

  • Collaborate with legal department to manage claims and litigation.

  • Implement and process outcomes for Cantex Continuing Care Patient Safety and Risk Initiatives, communication of results to Senior Leadership.

  • Promotion of Patient Safety/Loss Prevention and Reduction of medical errors.

Quality Assurance Performance Improvement (QAPI)

  • Lead and coordinate Quality Assurance Performance Improvement initiatives throughout organization: Providing an executive summary at Quarterly Quality Assurance meetings about Community-specific compliance with Quality Assurance, Safety Awareness, as well as Risk Management review of key systems that triggered

  • Establish and monitor Key Performance Indicators (KPI), quality and safety outcomes

  • Implement evidence-based practices to enhance patient care, operational efficiency

  • Assist in identifying learning needs and formulates goals/initiatives to improve skill development.
  • Conduct chart audits regularly to ensure adherence to quality standards - i.e. care rendered, desired outcomes achieved for reportable incidents, family complaints, care trend concerns, etc.…
  • Perform routine site visits to evaluate key risk areas for a Facility, Agency or Business unit, collects data, samples a resident population with identified areas of concern, provide verbal and written recommendations, and schedule follow-up visits to ensure risk areas have been adequately addressed.

Leadership and Collaboration

  • Quality Incentive Payment Program (QIPP) -Oversees, directs and ensures facilities attain the highest compliance possible while participating in the QIPP

  • Collaborate with Regional clinical team and facility leadership to maintain an acceptable Return to Acute that is better than State and National thresholds.

  • Collaborates with operational leaders to include planning, implementation, and promotion, scheduling, instruction, coordination and documentation of the on-boarding process.

  • Foster culture of safety, quality, and continuous improvement within the organization

  • Oversee data collection, analysis, and reporting related to risk management and quality improvement activities

  • Prepare and present regular report to senior leadership on risk and quality performance
