Director of Rehab / DOR needed for provider of skilled nursing / SNF. The ideal candidate will be a COTA / Certified Occupational Therapist Assistant / Occupational Therapist Assistant with management experience or strong geriatric background to be considered.
The COTA, Certified Occupational Therapy Assistant will be the primary representative of company at the facility, promoting policies and clinical programs to meet the customer???s expectations, to attract and retain employees, and to contribute to clinical improvement of patients/residents.

1.Associate???s degree as COTA, Certified Occupational Therapy Assistant
2.Current license in state of practice as a COTA, Certified Occupational Therapy Assistant
3.Excellent interpersonal, fiscal, and clinical skills
4.Computer proficient

VIEW JOBS 7/22/2018 12:00:00 AM 2018-10-20T00:00 In Home Therapy Clinician MA Behavioral Health Services Eliot Community Human Services Northeast In Home Therapy Team, provides intensive, community-based supports and therapy to children and families in crisis is seeking a Master Level Clinician. Clinical supervision hours towards licensure is provided. Responsibilities: Provide in-home, therapeutic interventions including individual, parent/child, and family systems solution-focused treatment. Conduct a comprehensive bio-psychosocial needs assessment for the child or adolescent and their family, which responds to the diversity of the individuals and families served. Take the lead in the development of a strength-based treatment plan and safety plan. Such plans will include behavior modification therapy and crisis intervention to determine ways to best defuse a situation without necessarily moving a child and linkage to needed supportive services. Provide emergency response and intensive outreach to families in order to assist them in stabilizing children and adolescents within their home during a psychiatric crisis. Be flexible with work schedule to accommodate the needs of the families during evenings and weekends. Meet established productivity target. Collaborate with Emergency Services as needed, regarding clinical alerts or crisis response plans. Able to utilize innovative, evidence-based techniques and motivational counseling to inspire families and to increase treatment plan compliance. Guide the delivery of services, formulate treatment, and communicate regularly with the IHT Therapeutic Training and Support Worker concerning on-going client/family needs and progress. Coordinate treatment planning and aftercare services with other treatment providers, so as not to duplicate services and to foster seamless and consistent support. Participate in an inter-agency Systems of Care Service Plan with involved state agencies, outpatient therapists, court personnel, primary care providers, educational advocates, and others involved with the family, when indicated. Attend discharge planning meetings for any referral from a hospital inpatient unit or CBAT program. Schedule: Monday-Friday (Varied Hours) Pay rate:$42,000 base, Incentive Program Requirements Master's Degree in Social Work, or related field, license eligible with relevant community-based social service work with parents and children. Able to provide in-home support. Ability to consistently meet all necessary background and credentialing requirements. Must have valid drivers license and reliable transportation to and from work and for providing services in the community.
