The Director of Access and Referral Management will be responsible for the strategic prioritization and implementation of system level processes and programs designed to maximize quality and efficiency while minimizing costs. In partnership with Rochester Regional Health (RRH) operational and corporate support divisions, this role will assure optimal patient care coordination and navigation. Key areas of focus include but are not limited to; provider-to-provider communication, provider to patient communication, patient access and lead times and, in-network utilization of services. Referral management is a key component of RRH's commitment to delivering high quality, clinically integrated care to the region.
STATUS: Full Time
Bachelor's Degree required.
Master's Degree preferred.
A minimum of seven (7) years of relatable experience.
Minimum of three (3) years of progressive leadership experience.
Utilizing best practices facilitates an integrated patient care delivery model that demonstrates ease of patient navigation with the ability to track services through completion.
Proactively prioritizes and implements sustainable processes as evidenced by provider and patient satisfaction, quality indicators, and in-network utilization.
Develop, monitor and maintain KPIs, standardized reporting, and analytics for Market Share, Access, and Leakage using tools including SPARCS, SG2, CareConnect and GRIPA claims data.
Develop a strategic framework to assess and prioritize RRH referral and access initiatives
Lead all referral management and access initiatives.
Spearhead projects across departments and coordinate key stakeholders/resources involved in referral management initiatives (call center, CareConnect, service line leaders, physician liaisons, etc.).
Partner with Service Line Leaders in applying analytics across all data sources to identify and drive market share/leakage reduction.
