The Clinical Programs Director is a key contributor to the Clinical Services team and focuses on optimizing key program initiatives for high risk complex patients. Two programs are prioritized under this role including Complex Care Team (CCT) and Transitions. Both programs intervene with high-risk patients to prevent unnecessary admissions, readmissions, and poor outcomes. In collaboration with the Medical Director for Complex Care Programs, s/he supports implementation and execution of the CCT program across all OSH Markets. The Director provides support for hiring, training and ongoing supervision of the CCT CIS/MAs in Illinois; coordination of CCT initiatives and staff enrichment; and collaboration with all corporate teams to optimize CCT (IT, Marketing, Business Intelligence, Population Health). This role provides valuable leverage to many clinical related projects, designed to improve patient outcomes, advance our organizational mission, and improve patient and provider experience. The Director acts as the operational lead with clinical partners across the clinical services team to optimize each program outcome.
Support development and implementation of CCT program, such as clinical guidelines, medication reconciliation training, EMR training and support, as well as other IT-related projects
Supervise 4 CIS/MAs in IL including hiring, training, maintenance of role profile, and coaching performance
Supervise one CCT Central Scheduler
Manage multiple project plans to ensure efforts stay on track for deadlines and prepare updates and read-out on project progress
Manage technology, dashboards and quality metrics related to the CCT and partner with the program director to seek opportunities for improvement
Support the program director with clinical home visits, including but not limited to: scribing visit notes, preparing schedules and supplies for appointments
Collaborate with all corporate departments and clinic teams to identify opportunities for improvement and implement solutions
Liaison between CCT operational leads in all markets
This role reports to the Vice President of Clinical Programs and currently has 5 direct reports.

Project management experience
Experience in healthcare operations preferred, but not required
Ability to travel regularly throughout Chicagoland visiting & working with our clinics
US work authorization
