Job Description:

We are seeking a dynamic and experienced Director of Case Management to lead our dedicated team at a state-of-the-art acute care facility in Jacksonville, FL. This pivotal role requires an individual with a strong executive presence, exceptional communication skills, and a proven track record of delivering results and achieving goals. The ideal candidate will be a registered nurse or licensed clinical social worker with extensive experience in hospital case management and leadership within a large acute care setting, preferably with trauma-level exposure.

Key Responsibilities:

• Operational Leadership: Direct and evaluate the operations of the case management department, ensuring optimal staffing, technology use, onboarding, and staff competencies.
• Resource Allocation: Strategically allocate resources to meet productivity and quality objectives effectively.
• Compliance Oversight: Ensure adherence to care coordination, discharge planning processes, and compliance with organizational policies and external regulations.
• Performance Management: Assess and enhance department performance through process evaluation, data analysis, and implementation of improvement activities.
• Stakeholder Engagement: Establish and maintain relationships with key stakeholders such as CMO, CNO, ancillary service leaders, Ethics & Compliance, and Legal departments.
• Professional Conduct: Uphold the organization's "Code of Conduct" and "Mission and Value Statement."

Qualifications:

• Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
• Active Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) required.
• 5-7 years of experience in acute care hospital case management.
• Minimum of 2 years in a case management leadership role within a large acute care facility.
