The Case Management process facilitates communication and Care Coordination along a continuum through effective transitional Care Management.  The Director of Case Management utilizes the following processes to meet the patient's individual healthcare needs: assessment, planning/intervention, implementation, Care Coordination, monitoring, evaluation of the plan of care and communication. The RN Director of Care Management is responsible for the achievement of optimal health, access to services, advocacy, appropriate utilization of resources and collaboration with post-acute care providers to ensure patient's needs are met in the community. The Director of Case Management utilizes the following processes to meet the patient's individual healthcare needs: assessment, planning/intervention, implementation, Care Coordination, monitoring, evaluation of the plan of care and communication. Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners, and the community.

Qualifications:

• Registered Nurse required  
• Bachelor Degree in Nursing required
•  Master’s Degree preferred 
• Director of Case Management in an acute hospital setting

Responsibilities:

• Collaborates with the oversight and management of the ministries Physician Advisor program to ensure appropriate medical staff engagement, education, understanding and compliance
• Directly supervises, manages and fosters professional development of staff
• Applies uniform standards of practice, supports the recruitment of qualified individuals, and provides ongoing education, evaluation and performance feedback as appropriate
• Annually develops and engages a strategic work plan that incorporates quality, clinical, administrative, and financial goals and objectives while delivering on the plan metrics
• Seeks to improve quality, performance, and outcomes
• Develops budget for all areas of responsibility, including staffing productivity and performance metrics
• Monitors and manages any variance against budget, with the expectation of meeting or exceeding planned performance
• Fosters collaborative communication and engagement with the medical staff and other community care partners to support optimal Care Transitions and patient experience
• Directly accountable for ministry Care Transitions related to Case Management, Utilization Management, discharge planning, and psychosocial interventions
• Identifies and acts on negative trends and variances
• Manages regular reporting and communication to appropriate ministry and regional stakeholders regarding Care Management performance and outcomes
• Actively seeks out consultation and support to/from executive leadership for challenges related to areas of responsibility, patient care, and patient outcomes
• Serves as a coach, mentor, educator and advocate for direct reports and medical staff
• Ensures appropriate licensing, regulatory and accreditation compliance
• Actively seeks to improve quality and care programming to meet the core objectives
• Acts as point and liaison for various community care partners that may be involved in Care Transitions and Continuum of Care pre/post-hospitalization
• Supports a culture of learning and excellence, that includes continuous professional development of staff
