Overview

We are searching for a RN with current or prior adult care home health experience in a management role to supervise, monitor, evaluate and provide direction to direct reports. Provide professional growth and development; develop and communicate annual goals and objectives; monitor and provide constructive feedback; conduct annual performance evaluations, and provide training.

Participates in strategic planning for the agency to promote growth, development, and implement short term and long-term goals. Demonstrates proficient knowledge of and adherence to state and federal regulations and intermediary guidelines; provides accurate interpretation of guidelines, and integrate guidelines and regulatory requirements into agency policies and procedures.

Responsibilities

Responsible for monitoring agency license and branch location; develops and prepares reports as requested to document performance improvement initiatives. Participates in community activities to create a positive visibility for the Agency.

Responds to any inquires from the community regarding the Agency and its services. Completes accurate and timely documentation. Evaluates documentation tools and revises as needed to ensure compliance with applicable state and federal regulations, payor/contract requirements, and accepted clinical standards.

Maintains current, proficient knowledge of clinical procedures; provides training and educational information to staff on clinical procedures updates. Responsible for program clinical and support services programs; conducts assessment of staff, patients, referral sources, and the community to determine need for appropriate services and programs. Responsible for performance improvement reviews of all clinical activities for the program to include chart audits, supervisory visits, and other oversight activities.

Ensures administrator/alternate administrators maintain required training hours. Performs visits to agency locations regularly to oversee operations. Performs all other duties as assigned.

Qualifications

Seeking a licensed registered nurse at least one year of management experience in a adult care home health setting.

Must have current CPR

Must have a valid driver license

Must be willing to travel to all of our locations within the service area (TX)

Experience with memory care residents is also preferred. A background in sales, business development & marketing is a big plus. Must be a PTA or COTA. The incumbent may be responsible for providing therapy services including assessment as applicable, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the discipline's qualifications, professional practices and ethical standards. The incumbent shall also be accountable for and contribute to program development, quality improvement, problem solving, community education, and productivity enhancement in a flexible interdisciplinary fashion. 1. Ensure for program staffing team supervision and development. 2. Monitor team members competency, licenses/certifications, and memberships in applicable professional organizations. 3. Coordinate the development of team members through structured activities. 4. Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others. 5. Promote adherence to policies and practices of applicable professional organizations, client SNF and RehabCare Group. 6. Contribute to client relations and community education activities. 7. Assures that all patient care activities are performed according to the company policies and procedures and all applicable state and federal laws and regulatory agencies. 8. Participate in coordination of facility quality management and program evaluation activities. 9. Participate in survey preparation activities 10. Interact with assigned program consultant. 11. Participate in facility safety committee activities. 12. Manage the delivery of care in the most cost effective manner. 13. Coordinate treatment activity between disciplines and team members. QUALIFICATIONS: A track record of successful management experience and successful patient care experience is preferred. Experience working with a geriatric disabled population is desirable.
