Job Summary:

Responsible for the programmatic development, planning, organization and system wide strategic direction of the inpatient departments and functions that focus on the management of patient outcomes. Functions include but are not limited to:

Inpatient Case Management, Social Work and Utilization Management. The Director promotes interdisciplinary collaboration with medical staff, nursing, hospital and system leadership, fosters teamwork and champions service excellence.

Responsibilities:

Demonstrates a broad knowledge of clinical practice using innovative critical thinking skills. Demonstrates expert knowledge of HCM functions including but not limited to working knowledge of medical necessity criteria, patient management, case management documentation systems, and all other systems used in the daily case management activities.

Mentors, supports, and develops Case Management Leadership team in their job functions and responsibilities and professional growth and development

Works collaboratively with Physician Advisor team to identify deviations from standard practice and to implement or develop interventions to support service recovery. Works with staff to mentor, advise and support effective change in esponse to identified barriers

Consult and collaborate with system directors, administrators, senior management, nursing and medical staff leadership in planning to meet patient needs; system goals & objectives to assure quality continuity of patient care and effective utilization management. Plans and leads implementation of new programs for the organization to support effective continuum of care.

Continuously assesses and improves the department's performance by designing operational processes, monitoring performance through data, analyzing the data, implementing sustainable performance improvement, and participating in multi-disciplinary, inter-departmental PI activities.

Plans, organizes, and directs the programs and operations of Hospital Case Management.

Responsible for financial management activities for the department to include the development and management of applicable budget processes.

Responsible for the management and reporting of all regulatory compliance efforts with various applicable regulatory agencies.

Requirements:

Education: Master's degree in Nursing or related field or Masters in Social Work.

Experience: At least 5 years' management experience within inpatient case management.

Number and Type of Employees Supervised: 2-5 direct reports. 101-150 indirect reports.

Licensure, Registry or Certification Required: Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the South Carolina Board of Nursing, or, LMSW (Licensed Masters Social Worker) or LISW (Licensed Independent Social Worker) (Lawson Code LLSW). Accredited Case Manager (ACM) or Certified Case Manager (CCM) preferred.

Special Training: Utilization Management, Regulatory and administrative environment in which the organization functions:

COP, TJC and others. Medical Necessity Criteria (Interqual, MCG) knowledge and experience. Case management; care coordination, quality indicators in a tertiary care hospital.

REPORTS TO: Medical Review Manager LOCATION: Missoula FSLA STATUS: Exempt POSITION RESPONSIBILITIES: The role of the Medical Review LPN Case Coordinator is to coordinate referral requests and to promote the quality and cost effectiveness of medical care by applying clinical acumen and the appropriate application of policies and guidelines to prior authorization and pre-determination referral requests. The Medical Review LPN Case Coordinator ensures that referrals are responded to and completed in timely fashion to meet contractual requirements, prior authorization and pre-determination requests are completed in a timely fashion to meet contractual requirements and that all reviews are conducted using nationally recognized and evidence based standards. Applies clinical knowledge to make determinations for referrals, pre-authorizations and pre-determination of specific procedures and services to ensure adherence to contract benefits and plan provisions. As needed, they will forward requests to the appropriate physician or medical director with recommendations for other determinations, ensuring that the member is receiving the appropriate quality care, while making sure guidelines are followed. The incumbent is expected to conduct all communication and assignments professionally with peers, supervisors, subordinates, vendors, customers, and the public. ESSENTIAL JOB FUNCTIONS: Essential job functions include the following. Other functions may be assigned as business conditions change. * Coordinates case referrals according to the established policy. * Prepares and presents cases for Medical Director and Peer Review according to established policy. * Conducts prior authorization and Pre-Determination activities and referral management activities. * Assesses medical necessity by screening available information against established criteria, using MCG, Clinical Decision Support Tool, NCCN, NIH, FDA regulations, behavioral health criteria and medical policy. Interprets information and makes decision whether authorizations fits the benefit plan language. * Ensures timely reviews for requesting facilities and providers. Appropriate notification to parties. * Contacts member and / or provider to obtain or clarify medical information as necessary. * Refers cases to Case Management, Care Coordination, or Disease Management for review as necessary. * Refers potential quality issues and complaints to SQIP Committee. * Notifies Corporate General Council and/or Administrative Procedures Department of cases for review of potential fraud. * Understand the importance of Risk Management and cost savings analysis for all group plans. * Assist Senior Management with special requests that pertain to the Medical Review Team. * Participate in administrative meetings with Allegiance Care Management customers, Account Representatives or other group contacts upon request as assigned by supervisor. * Responsible for maintaining confidentiality standards, as mandated by HIPAA. * Provides oversight for authorizing those services that require prior authorization and pre-Determinations, acts as a liaison with the established provider network. * Utilizes designated criteria along with clinical knowledge to make referral and authorization decisions and assist with review determinations. * Evaluates all requests for service, to determine the Company's financial liability including the collection of information regarding subrogation and COB and entry in the automated system. * Evaluates the pre-service authorization request received for ambulatory surgeries, outpatient services, durable medical equipment and medications. * Initiates and continues direct communication with health care providers involved with the care of the member to obtain complete and accurate information. * Ensures accurate coding using CPT-4 and ICD-10 codes and documents all information accurately. * Collaborates with Administrative Procedures to ensure appeal process procedures are followed. * Maintains current clinical knowledge and skills through the attendance of seminars, available Internet education, written works, or other similar resources on a biennial basis as evidenced by a documented 24 CEU hours during a two year renewal period as recognized by the State Board of Nursing. * Advises non-clinical staff on clinical and coding questions. * Meets service standards for decision turn-around times and written correspondence. * Performs other duties and responsibilities as assigned. * Contribute to the daily workflow with regular and punctual attendance NON-ESSENTIAL JOB FUNCTIONS: * Preparation of forms, faxing, copying, scanning and filing. * Phone calls requesting information for admissions. * Organization of files and reference materials. * Ability to operate typical office equiptment and working knowledge of basic office practices. * Utilization management and Case Management support and backup as needed.
