The RN Director of Care Management is responsible for the achievement of optimal health, access to services, advocacy, appropriate utilization of resources and collaboration with post-acute care providers to ensure patient's needs are met in the community. The Care Manager utilizes the following processes to meet the patient's individual healthcare needs: assessment, planning/intervention, implementation, Care Coordination, monitoring, evaluation of the plan of care and communication. Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners, and the community. The Case Management process facilitates communication and Care Coordination along a continuum through effective transitional Care Management.

Qualifications:

• Registered Nurse required
• Bachelor Degree in Nursing required
• Director of Case Management experience in a hospital setting required 
• CCM or ACM preferred and within 1st year of hire
• Master’s Degree preferred

Responsibilities:

• Evaluates the effectiveness of Case Management services in terms of reimbursement for inpatient care
• Coordinates, negotiates, and procures services and resources to manage the care of complex patients, with the aim of achieving high-quality and cost-efficient patient outcomes
• Seeks opportunities to reduce costs while maintaining the highest standards of care Develops clinically based Case Management, discharge planning and Care Coordination to ensure the appropriate utilization of resources across the Continuum of Care
