SUMMARY:

Provides and documents contract/grant requirement services to clients of program.

EDUCATION AND EXPERIENCE:

Education: Bachelor degree in mental health and/or social service related field preferred.

Licensure: N/A

Certification: Must meet requirements of contract/grant.

Experience: Experience in mental health field and/or intellectual disabilities preferred.

Knowledge/Skills:
Verbal/written communication skills.

Ability to maintain composure in difficult situations.

EQUIPMENT:

Computer, fax, copier and any other equipment required to perform the functions of the position.

MAJOR DUTIES AND RESPONSIBILITIES:

  • May provide direct services to individuals and families served by the program.

  • Responds calmly and appropriately in crisis situations, implementing effective therapeutic interventions and strategies.

  • Remains current in knowledge of community agencies and resources, both internally and externally, and how to access resources in order to provide information, referrals and a seamless system of care for individuals served by the program.

  • Coordinates services provided with all facets of the program.

  • Function as program representative within the community.

  • Maintain adequate record keeping. Required documentation will be completed timely and in accordance with contract and/or program requirements.

  • Communicates with supervisor regarding concerns/developmental opportunities and quality of services provided by the program. Staff will evidence no critical incidents resulting from a lack of client supervision.

  • Attend and participate in regularly scheduled staff meetings and inservices and individual program planning staffings as needed. Staff will evidence timely and successful completion of all training requirements.

  • All other duties as assigned.

Responsible for pre-employment activities, job development, job placement, on-site training and long-term follow along support to enable adults with severe disabilities to acquire and maintain employment in a community-based job. EDUCATION AND EXPERIENCE: Education: Bachelor's degree required. Licensure: N/A Certification: N/A Experience: Experience working with disabled adults in a vocational rehabilitation or supported employment setting preferred. Knowledge/Skills: Ability to maintain composure in difficult situations. Verbal/written communication skills. Ability to safely operate motor vehicle. MAJOR DUTIES AND RESPONSIBILITIES: * Demonstrates extensive knowledge of principles and practices of the assigned program/services. * Assists with the implementation of program goals and objectives through direct service delivery. * Adheres to all program/service licensing standards and requirements. * Addresses emergency situations, responds accordingly and assesses corrective/preventative plan of action * Provides assessment services to assist in the determination of eligibility, vocational strengths, interests, abilities, obstacles and other vital information related to acquiring employment. Individuals served will achieve or maintain his/her desired community based employment goal. * Provides individualized job development and placement services through techniques such as employer interviews and market surveys to identify potential employment sites for individuals served. * Provides on-site training and support to individuals and employers to maximize the potential for successful job placement. * Provides advocacy and long-term follow-along/support services to maximize the potential for job retention. * Acts as liaison between individuals served, employers, families/caregivers, funding source representatives, and significant others to facilitate communication and support to assist in the employment process. Must meet contract standards of the assigned caseload as indicated by contract requirements and established production targets re: successful job placement * Maintain accurate and timely documentation of all services in accordance with FHI guidelines, licensure/quality enhancement guidelines, and all funding source(s) guidelines/requirements. Records documentation must meet compliance with applicable standards as indentified by payor source and as measured by the Utilization Review form * Develop and maintain positive working relationships with other service providers/funding sources to ensure all necessary supports are included to maximize the potential for employment success for the individual being supported. * Attend and participate in regularly scheduled staff meetings and inservices and individual program planning staffings as needed. * All other duties as assigned.
