As a Caregiver (Direct Support Professional (DSP) with REM Wisconsin, you'll support individuals we serve in day and residential settings.

We have openings for full and part time positions of all shifts (days, evenings, overnights, and weekends) in the Madison area.

We seek a compassionate person to:

  • Assist with daily activities such as meal preparation and planning; medication administration; and personal care.

  • Support community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities.

  • Partner with other staff members to support individual service plans and goals.

Qualifications:

What we require of you:

  • Must be minimum of 18 years of age

  • Valid Driver's license with no suspensions due to moving violations in past 3 years

  • Access to a registered vehicle with proof of insurance

  • Must be able to lift minimum of 25 pounds

  • Successful clearance of background checks

VIEW JOBS 3/20/2019 12:00:00 AM 2019-06-18T00:00 The Caregiver provides direct support in-home services to one, two, or three individuals with intellectual and/or physical disabilities by promoting choice, personal growth, and independence in their homes and community. The Caregiver is expected to provide personal care, assistance in carrying out daily household routines, transportation (in some cases) and companionship. Assist with activities of community integration and related work as required. Position includes working weekends. Staff may work under the supervision of the Live-in Residential Care Worker, the house Team Lead, and/or the House Case Manager. The specific job duties for the Caregiver are designed around the needs of each individual consumer and may be modified as the needs of the clients change. Duties and Responsibilities include but are not limited to: * Provide instruction and assistance in daily activities and routines such as: * Meal planning and preparation. An evening meal will be prepared each night with consumers active participation and will include all four food groups (Nutrition Pyramid). * Personal care and appearance (e.g. bathing, toileting, transferring, dressing and exercising). * Maintain a clean and orderly home environment (e.g. assisting consumers in the completion of laundry and household cleaning). * Giving medications accurately and reliably. * Management and documentation of consumer finances. * Scheduling recreation activities and providing or scheduling transportation. * Provide problem-solving assistance and companionship. * Accompany the person into the community to engage in recreational activities and doctor appointments. * Maintain records and documentation as required by agency, county personnel, service brokers, guardians and family members. * Participation in job-related training's, house meetings and supervisory meetings as scheduled. * Assist individuals to develop and expand participation in typical community activities and build relationships with family, friends and neighbors. * All other duties as assigned Minimum qualifications required: * High school diploma or GED * Valid driver's license and reliable vehicle for houses requiring driving * Must have reliable transportation to and from work * Must be able to work independently with minimal supervision * Functional reading and writing skills * Ability to work flexible hours and unconventional schedules, including evenings, weekends, overnights, and holidays * Able to pass Care Giver background requirements per state law * Regular attendance and punctuality (may need to cover shift until relief worker arrives) Hours: (hours are subject to change) * Friday: 4pm – 8pm * Every other Saturday 2pm-10pm (2 openings) * E/O Sunday 2pm-10pm (2 openings) * Mondays: 5pm-10pm Integrity Residential Services Madison WI
