POSITION SUMMARY:

Direct Support staff assigned to the Independent Living Supports (ILS) program performs various services as directed by the Client's ILSP - Independent Living Service Plan. Direct Support Staff may provide services to the extent they have been trained and deemed competent by a program director, registered nurse or director of ILS program.

REPORTS TO: Program Manager & Program Director

ESSENTIAL JOB FUNCTIONS:

  • Provide supports to each Client assigned as per their ILSP - Independent Living Service Plan. Supports may include but are not limited to the cueing of or full or partial assistance with the following; however, they must be specifically indicated in the ILSP:

o Activities of Daily Living (ADL) - meals, bathing, dressing, grooming, work, recreational, social and leisure. o Instrumental Activities of Daily Living (IADL) - managing money, shopping, telephone use, cleanliness of personal property, preparing meals appropriate for dietary needs, and community integration.

o Personal Cares.

o Medically Orientated Tasks: must be trained on and accept delegation of from the MAPC RN regarding Clients that have delegated tasks in their doctor's orders or plan of care.

If not able or willing to accept delegated tasks written declination must be included in employment folder. Be responsible to verbally remind MAPC RN and supervisor of inability or unwillingness to provide

delegated Medically Orientated Tasks if inadvertently asked to do so.

o Challenging Behavior Management.

o Transportation and escort - professional appointments, shopping and outside activities.

o Daily Living Skills training.

o Ensure Client Rights are always met.

o Recognizing and responding to condition changes and/or emergency situations.

  • Meet professional expectations to include but not limited to:

o Adherence to principles of infection control and universal precautions as per training.

o Follow approved ILSP; respond appropriately to unapproved requests from the Client.

o Maintain accurate, timely and consistent documentation in Client records.

o Communicate timely and consistently whether verbal, written or electronic to appropriate individuals. Communication must meet the ability or expectation of the individual.

o Respect Clients, guardians, care management, funding sources fellow team members and management always. It is expected that direct support staff are approachable, competent, dependable, trustworthy, and consistent with the provision of high-quality supports.

o Prevention of, recognizing and reporting abuse, neglect and/or misappropriation of property.

o Maintain a high respect and adherence for professional boundaries, ethical behavior, and Confidentiality and HIPAA laws.

o Always use adaptive or personal protective equipment properly and safely.

o Responding to emergency situations including possible evacuation or shelter in place situations.

o Participate in performance improvement activities and efficient resource management.

o Be a reliable and competent team member regarding performance, teamwork, safety and adherence to schedule. o And other duties as assigned and trained

SUPERVISORY RESPONSIBILITIES:

  • None.

JOB REQUIREMENTS:

Reliable, consistent attendance is a requirement and essential function of all staff positions.

Education/Experience:

o Successful and timely completion of program related training requirements.

o Successfully pass a criminal background check upon hire and every 4 years thereafter.

Skills:

o General math, documentation, personal care, and community integration, and social interaction.

o Successfully complete return demonstrations of all Medically Orientated Tasks, on the job training and other trainings, as necessary.

o Safe handling, storing and preparation of well balance and nutritional meals.

o Direct Support Professionals expected to drive as part of their job duties will have an agreement in their employment file regarding expectations of the job duties.

EQUIPMENT, MENTAL OR PHYSICAL DEMANDS and ENVIRONMENT:

Equipment or Personal Protective Use:

o Personal Protective Equipment.

o Operate, activate, use, prepare, place, and/or position supplies and equipment needed for Client care. o Knowledge of how to operate typical household appliances, including stove, microwave, dishwasher etc. o Specialized adaptive equipment - training provided.

Mental performance and communication elements:

o Client and customer interaction through active listening, clear communication, identifying, detecting, deciphering, judging, observing, inspecting, distinguishing, and assessing.

o Ability to critically assess, solve and interpret situations, variables, or instructions.

o Ability to manage and interpret Client documents and plans of care.

Physical Requirements:

o Move, transport, position self, bend, stoop, kneel, crouch, reach, remain in stationery position, push, pull, lift, carry, and ascend/descend.

o Regularly lift and/or move up to 35 pounds

Working Conditions and Environment:

o Moderate noise level.

o High degree of contact with Clients and other personnel who support the Client.

o Will have exposure to Blood borne Pathogens and communicable diseases.
