Easterseals Blake Foundation is seeking direct professionals to work in a collaborative environment supporting individuals with disabilities in their homes and the community.

Supported Living Direct Professionals work directly with people in their homes and in the community, assisting them to develop a sense of identity, self-worth and independence.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Assistance in the following areas

  • Meal prep

  • Upkeep of home, through innovation, take daily chores/duties and turn them into interesting individual learning experiences.

  • Community activities, Plan and conduct incredible activities and everyday fun

  • Grocery Shopping

  • Bill pay

  • Appointments

  • Transportation

  • Social gatherings

  • Medication management

  • Personal hygiene

  • Function as an enthusiastic, competent, contributing member of EBF staff

  • Attend and participate in required staff meetings

  • Promote and follow practices that support health and safety of staff

  • Develop and maintain positive relationships with individuals we serve, team members, families and state agencies. Encourage positive team work and problem solve in a non-hostile manner

  • Complete paperwork within required timelines

  • Attend all necessary training within the time lines established at the time you were hired

  • Write in "in-house" communication log after every shift worked. Maintain and monitor daily communication log between day program and Supported Living program

  • Maintain agreed upon work schedule, be punctual, and use time effectively

  • Use supervision effectively, accept feedback in a professional manner, and accept responsibility for job performance

  • Communicate essential information with ESBF team leads and supervisors accurately and in a timely manner

  • Understand and maintain private and sensitive information within the confines of need-to-know parameters

KNOWLEDGE, SKILLS AND ABILITIES

  • Ability to work flexible hours including early mornings afternoon and evening hours and at least 1 weekend day;

  • Ability to speak and write effectively in English.

  • Sensitivity to cultural diversity and differences.

  • Ability to complete job responsibilities autonomously without daily oversight.

  • An ability to acknowledge, respect, and build upon the diversity of person served and their families with regard to such factors as race, ethnicity, family constellation, gender, socioeconomic status, values, and traditions.

  • An ability to teach others using anticipatory guidance, modeling, coaching, reflection, and role-play

  • Must be able to utilize Personal Protective Equipment (PPE), such as face mask, gown, goggles, and gloves, for any amount of time necessary to provide a physical barrier to protect the employee and others.

EXPERIENCE / EDUCATION / LICENSE REQUIREMENTS

  • Must be able to utilize Personal Protective Equipment (PPE), such as face mask, gown, goggles and gloves, for any amount of time necessary to provide a physical barrier to protect the employee and others.

  • At a minimum a high school diploma or equivalent

  • Provide and maintain Valid Arizona Drivers License, Registration & Insurance

  • Ability to maintain a valid Arizona Fingerprint Clearance Card at all times

  • Experience providing personal care and/or working with individuals with a developmental disability

Provide direct care and supports for individuals with intellectual and developmental disabilities in a group home environment supported with personal protective equipment and universal safety measures. Position: Direct Support Professional (Caregiver) General Position Description: This position is responsible for a variety of direct care giving activities in the Easterseals Blake Foundation Community Living Services programs. Under daily oversight of the site lead/site supervisor and administrative supervision of the Residential Coordinator. Performs a variety of work consisting of routine difficulty in providing services/supports to individuals with developmental disabilities. The nature of the services that range from various personal care activities to activities outlined in an Individual Support Plan (ISP) and/or Behavior Treatment Plan (BTP). Performs miscellaneous related work as required. The Direct Support Professional independently performs the following: Essential Duties and Responsibilities: * Provides the appropriate environment to assure the health and safety of the members supported * Adherence to all EBF CLS Policies and Procedures and all standardized EBF Policies and Procedures * Assists members in performing daily living activities, including but not limited to: eating, showering, bathing, toileting, dressing, shaving, combing hair, shampooing, brushing teeth, cutting nails, etc. * Assists members with household chores (involving many physical activities, including but not limited to kneeling, reaching stretching, bending, etc. and the use of household cleaners) * Safe transport and/or escorts members to medical, vocational, residential, asocial activities and runs member related errands * Administers medications and/or assists members in the self-administration of medication including non-prescription and prescription as prescribed by licensed medical specialists * Safely and effectively monitors consumer activities, with or without a reasonable accommodation. * Accurately and completely report time in and time out of work * Observes and assesses members behaviors to provide positive behavioral supports * Read and implement Behavior Treatment Plans/proactive strategies to enhance and reinforce member positive behaviors * Assess and provides behavior/physical management techniques in crisis situation and attains assistance if needed * Records and maintains records, progress notes, records daily activities in communication logs, maintains records as needed and may be require to write reports * Reads and implements members individual support plan (ISP), medical/medication directions, and formalized records opportunities * Reviews and implements approved policies, standards, services and procedures * Takes part in developing and implementing Individuated Support Plans or person-centered plans for assigned members and encourages directives are following pursuant to State standards * Participates in staff and administrative meetings as well as in-service trainings * Reports unusual incidents on proper forms and informs appropriate staff regarding incidents involving vocational, day, social, recreational, behavioral, personal living, and community services problems within required time-frames * Observes and reports changes in members mental/physical health to enable intervention/prevention of problems * Performs housekeeping tasks such as cleaning, cooking, laundering, making beds, yard work, and other household chores or tasks required to support/serve the member including physical management procedures * Acts as an advocate in resolving problems and securing needed services for consumers as well as ensuring personal choice, freedom, responsibility, and support for consumers * Develops and maintain working relationships with all team members, including various operational administrative staff, family members, guardians, fiduciaries, advocates, etc. * Maintain positive communications and relations with employees, members, guardians, contract entities, authorities, and EBF * Able to remain calm, non-defensive, and with a supportive attitude during crisis or potential crisis situations * Must be able to endure extreme outdoor temperatures and sudden temperature changes * Must be able to operate an agency transport vehicle (unless declared as a non-driver or a vehicle is not required at the site) * Performs other duties and assignments as required Required responsibilities and tasks * Daily priorities may change frequently and the position requires quick response time Minimum Requirements: * Clean criminal background check and the ability to obtain and maintain a level one clearance card * Operate desktop computer, Microsoft Office software applications, and other technology as needed * Read, write and speak effectively in English * Must complete all in-home training within the first 14 days of hire and all other required trainings within 90 days of hire * Must have a valid Arizona Drivers License, Registration and Insurance at all times * Must be 18 years of age or older for non-driving position. * Must be 21 years of age or older for driving position. * Age: If driving is required for the position, must be 21 years old with no more
