JOB SUMMARY

Under the guidance of the QIDP's and the Program Service Director, the Direct Support Professionals are responsible for the implementation, carrying out, and documentation of all clients programs and objective while ensuring competent interaction with your clients at all times.

DUTIES

  • Work in a designated program area according to a pre-set schedule.

  • Implementing, training and documenting on all program objectives for clients on your caseload.

  • Documenting the program that was taught on the appropriate data collection days and stating any comments that might contribute to the understanding of the client's progress on any given program.

  • Work together with responsible QIDP to ensure all needs are met of your assigned clients.

  • Assisting in the distribution of food trays/bowls and implementation of dining room training programs.

  • Assisting in the distribution of snacks at designated snack times.

  • Responsible for maintaining cleanliness of all program areas at all times.

  • Reporting all unusual signs or symptoms regarding the medical or behavioral status of clients to the Director of Nursing, Supervisor, and designated QIDP immediately.

  • Assisting in client transportation, inside and outside of the facility as needed.

  • Ensure you follow your clients designated 24 hour schedule at all times.

  • Responsible for completing Behavior Repo1is, when appropriate, and turning in these repo1is to the appropriate designated area.

  • Responsible for maintaining client personal care boxes in a neat and orderly fashion.

  • Responsible for communicating with Program Service Director and/or QIDP when programming supplies, clothing, etc. are needed for your clients.

  • Participate in daily inservices as required.

  • Responsible for giving a report of clients status to oncoming (shift change) DSP.

  • Responsible for ensuring Active Treatment and Competent Interaction are occurring with all clients on your team at all times as per facility policy and State and Federal Regulations.

  • Ensures Room Care in performed daily (at times stated on schedule) and appropriately as per facility policy and State and Federal Regulations.

  • Ensures general hygiene skills are performed daily on all clients on your caseload according to objectives (i.e., bathing, tooth-brushing, grooming, etc.)

  • Makes recommendations and requests on behalf of clients.

  • Works directly with clients, non-professionals, and professional staff.

  • Other duties as assigned.

EDUCATION REQUIREMENTS

Preferred as a minimum a High School Diploma or G.E.D.

EXPERIENCE

At least 1 (one) year experience in a Health Care setting is recommended, but not required.
