Assists consumers with in-home activities of daily living while encouraging them to maintain or improve their level of function and independence and to live with as much dignity as possible.

1.Essential Functions of job:

a. Follows Written Care Plan established by the Registered Nurse if applicable

b. Provides Personal Care which may include: bathing, shampooing, dressing, shopping, preparation, and serving of meals.

c. Assists the consumer with moving around including but not limited to: aid with getting in and out of bed, from their bed to a wheelchair, on and off toilet, in and out of the bathtub, seated position to standing with a walker, and other assistance moving around as needed.

d. Provides assistance with housekeeping, which may include: dishwashing, dusting, laundry, linen changes, sweeping/vacuuming, and cleaning rooms, windows (inside only), cabinets and appliances.

e. Interacts and engages in conversation with consumer.

f. Reminds consumer to take their medications.

g. Demonstrates appropriate safety measures when providing care.

h. Recognizes emergencies; follows emergency procedures as well as instruction provided by emergency personnel.

i. Reports immediately to the agency, upon each incident, all known information regarding consumer hospitalization and discharge from hospital.

j. Informs supervisor of any unexpected or unusual changes in the consumer's condition, behavior, environment, and family situation.

k. Maintains confidentiality of consumer records and related information in accordance with HIPAA and agency guidelines.

l. Encourages consumer's independence in performing life activities.

m. Establishes positive rapport with the consumer and their family.

n. Follows Universal Precautions and Infection Control procedures.

o. Notifies consumer and supervisor of unscheduled absences in accordance with agency policies and deadlines.

p. Completes and submits timesheets weekly and the schedule of consumer visits monthly.

q. Comprehends and follows all Agency policies and procedures.

r. Maintains up-to-date CPR, First Aid certifications and other compliance documents where applicable.

1.Other duties of job:

a. Accompanies consumer to appointments when authorized by your supervisor.

b. Assists consumer with sorting through boxes to eliminate clutter, removing rubbish, and garbage accumulation (animal and human waste excluded), and packing and unpacking consumer belongings when consumer changes residence, cleaning attic, basement or other storage areas to eliminate fire and safety hazards when specifically directed by supervisor.

c. Provides coverage for other Personal Care Workers when call offs occur, as requested by the supervisor.

d. Attends meetings, training, and in-services as required.

ESSENTIAL RESPONSIBILITIES: Engage the individual and establish rapport as evidenced by emphatic and active listening, communicating in a nonjudgmental way, recognizing addictive behavior. (20%) Recognize signs and symptoms of addiction with sensitivity to cultural and environmental differences. (20%) Assist in development and enhancement of a comprehensive recovery plan by identifying resources, including community programs and agencies, and overcoming barriers to that plan. (35%) Ability to recognize emergency and crisis situations. (10%) Educate the individual, family, and community about disease of addiction and the recovery process. (5%) Conduct self in an ethical manner by adhering to codes of ethics and standards of practice. (5%) The position is part of CIH's Addiction Medicine team and will work with the addiction medicine population. The team takes a multidisciplinary approach to getting individuals connected to recovery services as well as appropriate treatment modalities.
