  • SUMMARY OF JOB:

Responsible for the fiscal management, personnel management, planning, coordination, implementation and evaluation of the provision of home health services, product lines and programs. Responsible for day to day delivery of patient care and operational activities.

Assumes 24 hour accountability for operational functions and the direction of multidisciplinary patient care teams. Responsible for developing operational plans, developing, monitoring and overseeing budgets and interacting with staff and other departments to accomplish organizational goals. Responsible for assuring Agency compliance with regulatory, fiscal and legal requirements in area of responsibility.

Responsible for new program development, as appropriate. Responsible to fulfill the role of Agency Administrator in the Administrator's absence. Functions as a liaison in the community, representing the organization to external agencies.

RECRUITMENT REQUIREMENTS:

BSN required and Master's degree in business or health related field preferred. 3 years progressive supervision/ management experience in healthcare or community health required. 3 years experience in a home health setting required. Knowledge of reimbursement and regulatory issues regarding home care required.

Computer literate. Excellent customer service skills and positive leadership style.

Excellent communication skills. Ability to act autonomously.

Ability to function in a rapidly changing health care environment. Able to travel throughout all HRHS entities.

Able to travel to patient's home within service area. Able to demonstrate professional procedures in the home care setting.

Able to use office equipment; i.e. beeper, car phone. Ability to work flexible hours, with holidays and weekends included.

LICENSE AND REGULATORY REQUIREMENTS:

Current licensure in State of practice, Valid Driver's License, Current CPR Certification

VIEW JOBS 7/18/2019 12:00:00 AM 2019-10-16T00:00 Job Description The Licensed Vocational Nurse (LVN)/ Licensed Practical Nurse (LPN) in expanded scope clinics is an integral part of the clinic team inside of MinuteClinic. Under the supervision of a MinuteClinic Nurse Practitioner or Physician Assistant, the LPN/LVN will advance the evidence based practice in all patient encounters as they perform the major job duties and tasks within the scope of their training. The role is responsible for assisting patients and performing or assisting with all clinical services available at the clinics. The LVN/LPN will perform patient registration and intake tasks, will assist with wait room management and other tasks to enhance the patient experience. Clinical responsibilities to include phlebotomy and specimen collection services, vaccine injection, patient assessment activities during intake such as vital signs, and performing diagnostic tests such as EKG and Retinopathy. MAJOR JOB DUTIES AND TASKS: 1. Phlebotomy and other Medical Services a. Perform blood draws and sending samples to a reference lab for processing. b. Able to demonstrate proper collection techniques in accordance with the laboratory collection services policy and procedure manual. c. Maintains knowledge of laboratory tests and appropriate computer ordering codes. d. Process specimens maintaining specimen integrity according to individual test requirements. e. Collect and document vital signs on patients f. Preparing, administering and providing vaccination services. g. Collect hair and oral fluid specimen samples and send to a reference lab for testing. h. Operate an EKG device for non-emergency readings on patients. i. Capture images of a patient's retina by using a fundus camera. j. Perform CLIA waived point of care testing on patients. k. Perform additional medical services that fall within the licensure of a LVN/LPN under the supervision of a Nurse Practitioner or Physician Assistant. 2. Patient-focused activities a. Assist with patient registration, available to answer questions regarding MinuteClinic services b. Assist with managing patients in the waiting space and level set wait time expectations. c. Act as a chaperone if a patient or provider in the primary exam room requests one. d. Participate in community engagement outreach and initiatives to drive practice growth. 3. Clinic Operations Support a. Ordering and maintaining adequate supplies for the clinic. b. Keeping Clinical Rooms clean, uncluttered and suitable for patient care c. Follows universal precautions in cleaning up or disposing of any blood of body fluids. 4. Additional Duties a. Employee may be required to complete or be responsible for additional job duties as assigned by Nurse Practitioner, Senior Practice Manager, Senior Clinician or Area Director. Required Qualifications Education: * Graduate of a school of Practical or Vocational Nursing, approved by the State Agency and/or accredited by the National League for Nursing (Accrediting Commission (NLNAC)) at the time the program was completed by the applicant. Licensure/Certifications: * The LPN/LVN must have a full, active and unrestricted license as a Licensed Practical or Vocational Nurse in a State, Territory or Commonwealth of the United States or District of Columbia. * The LPN/LVN must also hold a certification in performing phlebotomy services. Experience: * Minimum of one year related work experience in a healthcare setting required. * Experience documenting within an Electronic Medical Record required. * Ability to take vital signs, perform phlebotomy blood draws and perform Point of Care Testing required. Knowledge, Skills and Abilities: * Demonstrates effective verbal and written communication skills * Ability to work independently * Demonstrates initiative, problem solving ability, adaptability and flexibility * Clinical skills: documentation of care, medication administration, infection control precautions, assessing patient educational needs, physical assessment, obtaining vital signs, care coordination.> Preferred Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
