Director, Care Transition

Job Purpose

  • The purpose of this position is to manage and increase a facility's census by establishing, developing and maintaining relationships with hospitals, physicians, insurers, and community organizations that generate resident admissions.

  • Conducts physical assessments and/or screening of potential residents.

Essential Duties and Responsibilities

  • In accordance to facility and district sales plans, must develop and execute tactics to maximize admissions of residents requiring a sub-acute level of care including regular contact with potential referral sources including hospitals, physicians, insurers, case management companies and health care agencies.

  • Maintains an 80% conversion ratio from referrals to admissions.

  • Serves as a key member of the facility sales team by assisting other members with coordination of sales calls to key accounts

  • Conducts industry analysis annually and local market analysis quarterly. Completes the business development tool (sales plan) weekly.

  • Ensures admission screening of potential residents, determines level of care, services required, equipment needs and insurance coverage, as appropriate.

  • Ensures company processes are followed to ensure the clinical and financial approval process in a timely manner.

  • Coordinates admissions with appropriate departments and staff

  • Directs efforts to the managed care industry by setting up appointments with case managers and explaining the services provided

  • Contacts local community agencies to generate a positive image and encourage word-of-mouth referral activities.

  • Maintains up-to-date knowledge of current regulations governing Medicare / Medicaid and private insurance company reimbursement processes.

  • Ensures all required records are maintained and submitted, as appropriate, in an accurate and timely

  • Ensures 80% of time is spent outside the building on census development in the local market.

  • Participates in weekly sales team meetings, reporting regularly on referral development and progress on targets

  • Expected to attend Sava University to obtain sales tactics and techniques to allow for success in the role

  • Perform other duties as assigned

Job Requirements

Qualifications

  • Bachelor's Degree in marketing or 4+ years of relevant work experience

  • One to Three (1-3) year's experience in health care, long-term care industry or insurance sales/marketing

  • One (1) year experience with proposal development process

  • Public relations experience, preferred

  • Registered Nurse Preferred, but position may be filled by LPN/LVN

  • RN must have one (1) year experience in patient/resident assessments; LPN/LPN must have a minimum of two (2) years experience in patient/resident assessments.
    Knowledge & Skills

  • Strong technical writing skills

  • Familiarity with medical terminology

  • Superior presentation skills - both one on one and in a group setting

  • Proficient in Microsoft Office Suite (Word, Excel, Powerpoint)

Location: Westchester, IL Office The Nurse Reviewer Associate is responsible for conducting preauthorization, out of network and appropriateness of treatment reviews for diagnostic imaging services by utilizing appropriate policies, clinical and department guidelines. Primary duties may include, but are not limited to: * Collaborates with healthcare providers to promote the most appropriate, highest quality and effective use of diagnostic imaging to ensure quality member outcomes, and to optimize member benefits. * Receives preauthorization requests from front-line intake. * Conducts initial medical necessity clinical screening. * Determines if initial clinical information presented meets medical necessity criteria or requires additional medical necessity review. * Conducts initial medical necessity review of out of network preauthorization requests for services requested outside of the client health plan network. * Notifies ordering physician or rendering service provider office of the preauthorization determination decision. * Documents the results of the initial clinical review and determination in the precertification computer system. * Maintains confidentiality of patient and provider specific information. * Develops telephone service standards to meet departmental performance metrics. The Nurse Reviewer l is responsible for conducting preauthorization, out of network and appropriateness of treatment reviews for diagnostic imaging services by utilizing appropriate policies, clinical and department guidelines. Collaborates with healthcare providers, and members to promote the most appropriate, highest quality and effective use of diagnostic imaging to ensure quality member outcomes, and to optimize member benefits. Works on reviews that are routine having limited or no previous medical review experience requiring guidance by more senior colleagues and/or management. Partners with more senior colleagues to complete non-routine reviews. Through work experience and mentoring learns to conduct medical necessity clinical screenings of preauthorization request to assess assessing the medical necessity of diagnostic imaging procedures, out of network services, and appropriateness of treatment. Primary duties may include, but are not limited to: * Conducts initial medical necessity clinical screening and determines if initial clinical information presented meets medical necessity criteria or requires additional medical necessity review. * Conducts initial medical necessity review of exception preauthorization requests for services requested outside of the client health plan network. * Notifies ordering physician or rendering service provider office of the preauthorization determination decision. * Follows-up to obtain additional clinical information. * Ensures proper documentation, provider communication, and telephone service per department standards and performance metrics. The Nurse Reviewer Associate requires: * AS/BS in nursing; 1 year of experience; or any combination of education and experience, which would provide an equivalent background. * Current unrestricted license in applicable state(s) required. * Familiarity with managed health care programs including HMO, PPO, and POS and with ICD-9 and CPT 4 coding preferred. The Nurse Reviewer l requires: * AS/BS in nursing; 3 years of clinical nursing experience in an ambulatory or hospital setting; or any combination of education and experience, which would provide an equivalent background. * Current unrestricted RN license in applicable state(s) required. * Requires familiarity with Utilization Management Guidelines, ICD-9 and CPT-4 coding, and managed health care including HMO, PO and POS plans. * Previous utilization and/or quality management and/or call center experience preferred.
