Location:1 FTE -Buckley SFB, COService:Dietitian (Health-to-Health Registered Dietitian)PlaceofPerformance:The work to be performed under this contract will be at:Any of the MTFs in the Colorado Military Health System (CMHS), to include the United States Air Force Academy, CO; Peterson Space Force Base (SFB), CO; Schriever SFB, CO; Buckley SFB, CO; Fort Carson, CO; and/or all sub clinics associated with the CMHS, which includes locations in Pueblo County, CO and Tooele County, UT.

1 FTE -Dietitianinitial assignment will be at Buckley SFB, COWhile each HCW will be assigned to a primary duty location, the Government retains the right to assign an HCW, permanently or temporarily, to another location within a 40-mile commuting radius of their assigned duty location within the locations. Typically, HCWs will receive notification at least five (5) days prior to changing their assigned duty location.

The Government reserves the right to change the duty schedule for any HCW with 24-hour notice due to emergent events (e.g., surgery rescheduling, etc.).HCWs will not be required to commute more than 40 miles outside of their assigned duty location unless an allowance for travel is specifically authorized via bilateral modification.Any required contractor submittals for Qualifying, Credentialling, Privileges, and Security for a potential or actual assignment at another duty location under Task Order PWS sections 5.2.6 and 5.2.9. will likely be the same or nearly the same as what was originally provided for the initial duty location.

Therefore, these submittals must be provided to the Government within 3 days of request in preparation for potential or actual assignment at another duty location under this PWS.Telework:All labor categories under this task order will be required to perform services in-person. In emergent situations where mission essential services are required to be performed from an alternate location, telework may be authorized if government capabilities allow and the Contracting Officer approves.

This situational telework may be approved in accordance with Department of Defense Instruction 1035.01, Telework Policy, and the MTF's related policies.RecognizedHolidays:Contract HCWs will be required to work on federally recognized holidays.This is position and location dependent. Due to the critical nature of the required services, some HCW may be required to provide routine or emergency services on federally recognized holidays (or the actual day set aside for observation), to include New Year's Day, Martin Luther King Jr.'s Birthday, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, Christmas, and other executive ordered days off.HoursofPerformance:The performance (duty) hours of the MTF are as follows:Duty hours will vary by position and location; however, typical duty hours are scheduled between 0700 -1700, Monday through Friday. Hours are subject to change and may include nights and weekends.

In the event the HCW is required to work beyond their scheduled duty hours for any reason, the duty hours will be adjusted within the current pay period; the HCWs shall not work more than 40 hours per week. The performance (duty) hours of the MTFs are as follows: 10 MDG, United States Air Force Academy Monday through Friday, 0730-163021 MDG, Peterson/Schiever Space Force Base, Monday through Friday, 0730-1630.460 MDG, Buckley Space Force Base, Monday through Friday, 0730-1630.Evans Army Community Hospital is open 24 hours a day, 7 days a week. It is generally anticipated that the HCW(s) will work from Monday through Friday between the hours of 0730-1630.

However, duty hours will vary by position and/or location. Evans HCW(s) may berequired to work nights and/or weekends. HCW(s) shall not work more than 40 hours per week.Scheduling:The schedule or scheduling process is as follows:The Government will provide the HCWs with a specific duty schedule for each two-week period, at least five (5) days in advance.

The Government reserves the right to change the duty schedule to accommodate patient care needs and mission requirements with five (5) day notice. The Government reserves the right to change the duty schedule for some labor categories with 24-hour notice due to emergent events (e.g., surgery rescheduling, etc.).The HCWs shall schedule planned absences with their Government supervisor as early as possible, but no less than thirty (30) days in advance of the absence, to ensure that clinical operations are not adversely affected.The Contractor shall inform the Government supervisor and COR of any unplanned absence of an HCW (e.g., illness or emergency) as soon as possible, but no later than one (1) business day after the Contractor learns of the absence.Recording Time: HCWs shall input time into the Defense Medical Human Resources System -Internet (DMHRSi), or other designated system, to meet established standards of the MTF, to include, but not limited to, timeliness, accuracy, content, and signature.The HCW will also be required to track hours worked in Matrix' timekeeping system, Costpoint.Closures:

During a planned closure of the facility due to training, federally recognized holiday, etc., HCW services are not required, and the Contractor shall not invoice the Government for those hours.In the event of a training day, contractor staff may be required to participate in the training at the discretion of the government supervisor.During an unplanned closure of the facility due to unusual and compelling circumstances such as natural disasters, military emergencies, severe weather, etc., the Contractor will be compensated only or the actual hours the HCW provided services.On occasion, the Government may offer HCWs a shift at another CMHS location when the HCW's current duty location is experiencing an unplanned closure. HCWs are not required to take alternate shifts in this instance.

MissionEssential:Thislaborcategoryismissionessential.Due to this mission essential status, HCWs shall report to their assigned MTF if the MTF is open and functional, including during a crisis declared by the Secretary of Defense or the Overseas Combatant Commander.Legal Authorization to Work in the United States:HCWs performing under this task order shall have proof of U.S. Citizenship (certified copy of birth certificate or naturalization papers required). However, legal aliens who possess a green card or H1B visa, who are in compliance with the U.S.

Immigration Laws, and who meet the qualifications required herein may perform under the task order. In addition, the Government shall not be held liable for not allowing performance under this task order by a contractor employee who fails to comply with, or to maintain, the citizenship or legal alien status requirements stated herein.PeriodicProgressMeetings: Contractorwillberequiredto attendperiodicprogressmeetingsatnoadditionalcosttothe

Government.The terms at which the periodic meeting will be conducted include: Periodic progress meetings will be required only if performance issues result in failure to meet requirements defined in the TO.

Should the Government determine these meetings are necessary, they may be conductedvirtually.ContractorTravel:Reimbursementwillnotbeprovidedfortravelwithina40-mileradiusof the place of performance.All HCWs are subject to working at any duty location identified under Place of Performance. On occasion, the Government may determine that travel outside of the assigned MTF and/or off the military installation is necessary for a patient's medical situation and/or coordination of transitional care for Active-Duty Military or Wounded Warriors. If these instances require travel outside of a 40-mile radius of the HCW's assigned duty location, funding will be authorized and approved via bilateral modification.HCWs shall not travel outside of the 40-mile radius prior to the appropriate funding being obligated.

The contractor will be reimbursed for actual costs incurred up to the limits set forth in the DOD Civilian Personnel Joint Travel Regulation. The website for per diem rates is http://www.defensetravel.dod.mil/.ComputerSkillCompetency:EachHCWshalldemonstratecompetencyasrequiredinthe contractand as follows:The Government will provide all MTF computer training in accordance with MTF-specific policy. Each HCW shall be able to operate a personal computer utilizing Microsoft applications, such as Word and Excel, and efficiently utilize an Electronic Medical Record (EMR). Each HCW shall effectively learn, implement, and demonstrate computer competency in conjunction with performing accurate and complete medical documentation of patient care utilizing designated MTF computer systems in accordance with applicable Joint Commission, MEDCOM, Departmental, and Clinic policies.

Training shall be accomplished within two (2) weeks of on-boarding. The COR will coordinate computer training in conjunction with the in processing of the HCW. The Government will provide computer training to each HCW during normal duty hours.HCW Health Requirements:

The Contractor shall provide documentation certifying health requirementssuchasimmunizations,annualvaccinations,medicaltesting(i.e.,tuberculosis,N95 particulate respirator duckbill mask fitting) and physical examination when required at the time of initial placement.OccupationalHealth:TheHCWshallberequiredtoobtaindocumentationofrequiredphysical testing or a report of a physical examination.TuberculosisScreening:Theadditionalimmunization/screeningrequirementsfortheriskof exposure to tuberculosis (TB) are as follows:The HCW shall provide proof of a negative TB skin test within 12 months prior to the start of work. If positive, the HCW shall provide proof of a negative chest X-ray within 12 months prior to the start of work. After start of work, the Government will provide educational post blood borne exposure protocols.

All HCWs shall obtain the Hepatitis B Immunization unless exempted by a physician. Immunization information may be tracked in DoD computer systems for all HCWs. The Government will not provide any tests or procedures required by this task order at the MTF.

Expenses for all required tests and/or procedures shall be borne by the Contractor at no additional expense to the Government.The HCW shall be able to safely execute the responsibilities of their specialty in providing health care. Has no medical or mental condition that prevents or that significantly reduces an individual's ability to safely execute their approved scope of practice in providing health care. This includes alcohol or drug abuse, or mental health disorder not responsive to treatment.

LaborCategory:Dietitian (Health-to-Health Registered Dietitian)Qualifications:Degree/Education: Bachelor's degree.Graduate from a college or university accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) or completion of a Didactic Program in Dietetics (DPD) accredited by the ACEND or completion of a post-Baccalaureate degree Dietetic Internship Program of supervised practice (for a minimum of 1200 hours) accredited by ACEND.

In addition, the HCW shall also complete the online Air Force Health Promotion (HP) Orientation within six (6) months of hire/placement and attend the in-residence Air Force HP Workshop within 18 months of hire/placement.CertificationsinadditiontoBasicLifeSupport:NoneExperience:One year of specialized experience in diet therapy after graduation.Licensure/Registration: Current, full, active, and unrestricted dietetics license or certification as required in the TO.Possess current registration as a Registered Dietitian from the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.HCWDuties:ThedutiesfortheHCWare as follows:The HCW will be providing care under the Health-to-Health (H2H) Registered Dietitian program to include but are not limited to the following:•Plan and organize work of considerable difficulty and responsibility under minimal supervision.•Participate in research and performance improvement studies.•Conduct nutrition assessment/evaluation which includes nutrient intake, activity level, appetite, intake of vitamins, minerals, nutritional supplements, and other complimentary alternative medicine usage, weight history, taste changes, feeding problems, food intolerance, food-drug interactions, unhealthy diet behaviors, socioeconomic and ethnic background, documented medical history, current diagnoses and medical treatment modalities, current drug therapy, and clinical signs and symptoms of nutritional deficiencies. •Apply the basic principles of enteral and parenteral nutrition support.•Apply principles and practices of clinical dietetics and medical nutrition therapy in order to plan, develop, and independently coordinate medical nutrition therapy intervention, interdisciplinary care plans, and nutrition education activities.•Conduct studies, analysis, and document findings to evaluate staff, food, nutrition supplements/feeding or meal alternatives and therapies, supplies, equipment, and service in order to improve the efficiency of the operation.•Apply current food service systems management, principles, and practices within a healthcare environment.
