SUMMARY

The Dietitian role is to assure quality nutritional care of patients and their families with chronic renal disease.

RESPONSIBILITIES:

  • Assist with clinical and operational processes to improve patient health and minimize hospitalizations and missed treatments.

  • Actively participate and assist with the pre-ESRD program.

  • Demonstrate effective use of supplies.

  • Perform duties as assigned to meet the patient care or operational needs of the clinic.

  • Recommend therapeutic diets in consideration of cultural and/or personal preferences and changes in treatment based on the patient's nutritional needs in consultation with the patient's physician.

  • Answer questions and discuss problems with dialysis patients when excessive weight-gain occurs, or when other instances of suspected dietary non-compliance occurs.

  • Design and distribute educational materials concerning diet for the dialysis patient, conduct seminars and classes for dialysis patients concerning aspects of their diets.

  • Perform regularly scheduled evaluations of standard menus for renal patients, revising and/or updating menus as necessary, and participate in monthly interdisciplinary patient care conference.

  • Assist patients in obtaining dietary supplements as appropriate.

  • Provide ongoing monitoring of data to determine need for timely intervention and follow-up.

  • Assist with program's target goals for patient outcomes in accordance with quality patient care and Company goals.

  • Knowledge of and comply with federal, state, local laws and regulations, including health care professionals practice act requirements.

  • Perform duties at all times within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations.

  • Must perform initial assessments in accordance with state regulations.

  • Nutrition reassessments shall be conducted in accordance with state guidelines or more often if indicated.

  • Participate as a member of the Interdisciplinary team in all required patient assessment and care planning activities.

  • Help maintain a productive quality assurance program and participate in the center's quality assurance activities.

  • Assure follow-up progress notes are written in each patient's records each month.

  • Assure documentation corresponding with the patient's care plan is noted on a monthly basis or as needed.

  • Use personal protective equipment as necessary.

  • Be familiar withall emergency operational procedures.

  • Counsel patients, and patients' families on prescribed diets and monitoring adherence and response to diet therapy.

  • Refer patients for assistance with nutrition resources such as financial assistance, community resources or in-home assistance.

  • Attend and participate in the monthly Quality Assessment and Performance Improvement (QAPI) activities.

  • Maintain collaborative working relationship with Medical Director and physicians.

  • Establish and maintain a positive relationship with area nursing homes, hospitals, agencies, vendors and the community as applicable.

  • Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization.

  • Participate in staff meetings as required. Attend in-service and continuing education offerings in compliance with company policy and procedure.

  • Educate staff and provide inservice training in relation to patient dietary care.

  • Lead staff in team concepts and promote a team effort.

REQUIREMENTS

  • Must have Bachelor of Science Degree in Dietetics, Nutrition, or Food.

  • Minimum of one year experience in clinical nutrition as a Registered Dietitian required.

  • Current Registered Dietitian (RD/RDN) certification showing proof registered with Commission of Dietetic Registration.

  • Current Dietitian (LD/LDN) license in applicable state is required unless employed in the state of AZ, CA, CO, MI, or NJ. State certification (Certified Dietitian/Nutritionist) required in CT, IN, NY and VA. License must be maintained as current and in good standing.

  • Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.

  • Must have basic computer skills, including Microsoft Office (Word, Excel, Outlook); proficiency in all USRC clinical applications required within 90 days of hire.

  • Must meet any practice requirement(s) for the applicable state.

VIEW JOBS 11/9/2018 12:00:00 AM 2019-02-07T00:00 The Patient Care Technician, under the direction and supervision of the Registered Nurse, assists with providing care and delegated activities of treatment to patients with renal failure. This position performs duties as assigned by the Charge Nurse and Registered Nurse. RESPONSIBILITIES: * Assist with clinical and operational processes to improve patient health and minimize hospitalizations and missed treatments. * Demonstrate effective use of supplies and staff labor hours. * Perform duties as assigned to meet the patient care or operational needs of the clinic. * Obtain blood samples for laboratory analysis as required to complete physician orders. * May reinforce patient education as needed under the direction of the Administrator or Registered Nurse. * Assist with program's target goals for patient outcomes in accordance with quality patient care and Company goals. * Knowledge of and comply with applicable health care professionals practice act requirements. * Perform duties at all times under the supervision of a Charge Nurse and within limitations established by and in accordance with company policy and procedures, applicable state and federal laws and regulations. * Use appropriate safety measures including personal protective equipment as necessary. * Clean, disinfect, and rinse dialysis machines in accordance with policy and procedures. * Prepare machines for hemodialysis treatments. * Verify/respond to visual and audio alarms on machines; troubleshoot machine alarms as appropriate. * Remove disposables and clean station after each treatment. * May be required to clean isolation room post treatment if applicable. * Be familiar with emergency equipment and all emergency operational procedures. * Obtain pre and post treatment weights of patients per protocols. * Safely initiate and terminate dialysis treatments per center protocol, and within scope of practice and as allowed by state regulations. * Monitor patients during dialysis treatments. * Mix acids and bicarbonate according to protocol. * Responsible for accurate documentation of information related to patient treatment. * Stock hemodialysis carts and helps maintain supply room. * Complete water testing on dialysis machine at the beginning of each treatment day and as directed by the Administrator. Complete/document quality control logs throughout the day and after all treatments, as required. * Take vital signs per protocol during patient treatment and document on treatment record as required. * Report any significant information or change in patient condition to the Charge Nurse. * Report machine problems to Biomedical Technician and Administrator. * Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization. REQUIREMENTS * High school graduate or equivalent. * CPR certification required within 90 days of hire or as required by the state. * Confirmation of ability to distinguish all primary colors. * Prefer a minimum of 3 months in dialysis healthcare or phlebotomy experience or CNA experience. * Must be certified under a State certification program or a national commercially available certification program as follows (see addendum for Patient Care Technician)- * Certification must be obtained within 18 months of hire date as Patient Care Technician. Time employed as a Patient Care Technician prior to employment with USRC will count towards the 18-month deadline for certification, unless a gap in employment as a Patient Care Technician is more than 18 months. * Certification must be maintained as current and in good standing. * Successful completion of USRC training program approved by the Medical Director, including demonstrated competency, within 8 weeks of hire. * Successful completion of USRC training course in the theory and practice of hemodialysis within 8 weeks of hire. * Demonstrated working knowledge of the English language and ability to communicate verbally and in writing. * Must have basic computer skills; proficiency in all USRC clinical applications required within 90 days of hire. * Must be able to organize time and tasks well. * Noise level in the typical work environment is moderate due to the number of staff in business office area and staff/patients/equipment in patient care areas. * Exposure to chemicals used within the facility (Renalin, Bleach and Acidified Concentrate; refer to MSDS for other chemicals used within the facility). * Exposure to blood and other potentially infectious materials (ongoing training provided for employee safety). Maryland * Must possess current Certified Nursing Assistants – Dialysis Technicians (CNA-DT) certificate from Maryland Board of Nursing OR * Hired at USRC location recognized by state of Maryland as an approved dialysis technician training program and obtain Maryland CNA-DT certificate within three (3) months from date of hire OR * Out-of-state applicant must have active BONENT certification and provide proof of initial application for CNA-DT certification. New Mexico * Must have New Mexico dialysis technician certificate at time of hire OR * Hired at USRC location recognized by state of New Mexico as an approved dialysis technician training program and obtain New Mexico state certification within six (6) months of successful completion of training program OR * New Mexico certification of hemodialysis technician is required for out of state applicant with active state or national hemodialysis certification; must obtain New Mexico state certification prior to working as a Certified Hemodialysis Technician. * National hemodialysis certification is not required in New Mexico. * Hemodialysis technician students must have a current basic life support credential prior to and while the student has contact with patients. Ohio * Must have Ohio dialysis technician intern certificate at time of hire OR * Hired at USRC location recognized by state of Ohio as an approved dialysis technician training program and submit application for Ohio dialysis technician intern certificate to board no later than four (4) weeks prior to completion of the approved training program. * Must obtain Ohio Board of Nursing Certificate (OCDT) not less than twelve (12) months and not later than eighteen (18) months of enrolling in the USRC OCDT program. Oregon * Must possess current Certified Hemodialysis Technician (CHDT) certificate from Oregon Public Health Division at time of hire OR * Must have Oregon dialysis technician provisional certificate at time of hire and obtain Certified Hemodialysis Technician (CHDT) certificate from Oregon Public Health Division within twelve (12) months.
