This is a Part-Time or PRN position.
The Dietitian is responsible for assessing nutritional needs and providing nutrition care for all residents as needed and works closely with the Dietary manager, Physician and interdisciplinary team to recommend and implement nutritional care.
Complete initial and periodic nutrition assessments for all residents.
Work with the Dietary Manager to Coordinate the RAI processes to include MDS 3.0 and CAAs as needed
Write individualized resident care plans and update them as needed in collaboration with the Dietary Manager
Collaborate with the facility IDT and recommend nutrition related interventions to improve/maintain resident's nutritional status.
Assess Residents with increased nutrient needs and make as needed and review "at risk" Residents monthly to include but not limited to those with skin deficits, weight loss/gain, enteral nutrition requirements, hemodialysis and provide supporting documentation in the medical record.
Advise the facility on all nutrition related issues.
Monitor practices that promote and maintain sanitary practices that meet or exceed regulatory requirements.
Maintain current knowledge about nutrition.
Communicates with the Regional Registered Dietitian on all pertinent nutrition related issues
Communicates with the Regional Chef on any dietary operations issues.
Graduate of an accredited dietetics or nutrition program at the bachelor's level.
Registered by the Commission on Dietetics.
Licensed by the WV Board of Licensed Dieticians.
Completed at least 900 hours of supervised dietetics practice under a registered dietitian or nutrition professional.
Proficient with computers; prefer experience with Point Click Care (PCC) and Microsoft Office.
Maintains the confidentiality of resident's information
Honor resident's personal and property rights
Understands all rights afforded to residents and acts accordingly
