General Purpose of Job:
Provide nutrition care services to patients and clients by screening, evaluating and implementing care plans, providing diet education according to Food and Nutrition Services policies, procedures and standards of practice, in conjunction with medical and ancillary staff services.
This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
Screen patients for nutrition risk; collect relevant data, review of the medical record and patient/family data; determine risk level and intervention and document plan
Perform weekly screen for specialized areas and refer patients at risk; perform appropriate follow up on patients identified at low/moderate risk.
Perform nutrient intake analysis; initiate calorie count studies; calculate nutrient intake data per standards; document results; calculate specialty analysis and submit reports.
Provide pertinent information regarding food concerns, eating difficulties and other nutritional problems; help coordinate special patient requests/needs and facilitate diet order implementation; serve as a resource to units and diet office; provide patient referrals to dietitian.
Counsel patients on formula preparation, food/drug interactions or nutrition education; identify and educate patients on food choices for specialized diet programs to include weight loss, cardiac, fluid and sodium points.
Helps educate patients and client about the connection between food, fitnes and health.
May teach basic health and nutrition classes.
Collect and input data for research, PI or patient care; compile monthly report regarding patient care activities; develop forms, nutrition monitoring tools or education materials to assist in the management of patient care issues.
Participates in training dietetic students and staff, and will attend clinical meetings, in-services, and area-specific meetings.
Participates in weekend/holiday rotation.
Performs other duties as needed.

Work Experience Previous experience in health care environment. 1 year clinical experience or food service experience. Preferred Education Completion of Dietetic Technician Program approved by Accreditation Council for Education in Nutrition and Dietetics (ACEND), or BS in Dietetics or Nutrition , or a BS or BA degree with completion of all clinical nutrition courses required by ASCEND to apply for a dietetic internship, or a BS or BA degree with commensurate experience in nutrition and special diets. Specialized Knowledge and Skills Knowledge of nutritional food values, meal planning and therapeutic menu processes. Handling/gripping, speed movements, fine dexterity, writing and speaking skills necessary for the job, able to operate calculator, computer, typewriter, copy machines, and telephone. Must be able to communicate effectively with others, collaborating and accepting direction, able to work calmly and courteously when under pressure, display a positive and professional demeanor to patients, family members, and co-workers, able to be consistent in enforcing Sparrow Hospital Policies. Ability to speak and write in English fluently. Ability to speak Spanish desirable. Ability to manage multiple projects simultaneously, make judgements, make and follow through on plans, reasoning and problem solving skills, mathematical skills, organize and prioritize workload, attention to detail, memory. Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job.
