Primary Purpose:

This position is part of the Morning Pointe Operations Team, a resident-focused, high performance, high commitment work team that supports the Morning Pointe purpose of "partnering with families in meeting the challenges of life". The food service staff is responsible for making sure that quality food and beverages are served in accordance with Morning Pointe Policies and Procedures and local food service regulations.

Qualifications/Skills Needed:

The successful candidate must meet educational requirements by having a high school diploma or equivalent. Experience in food preparation work in a service environment is preferred. Strong verbal and written communications skills are essential. Demonstrated support and understanding of the philosophy of Morning Pointe and the five domains of health (physical, emotional, spiritual, intellectual, and social) is required. A flexible work schedule is also a necessity.

Duties include, but are not limited to:

  • Work with the entire Morning Pointe team to build relationships and strong communication with residents and families to ensure adjustment to the community, identification of needs and problems, and the development of an appropriate plan of service for each individual resident.

  • Verify meal request of residents and communicate same with cook.

  • Brew coffee and tea and serve as requested. Serve water and juice as requested.

  • Assist with serving meals as directed by supervisor.

  • Promptly bus tables after meals and reset tables for next meal with silverware/stemware/coffee mugs/cloth napkins. Replace clean tablecloth toppers when needed.

  • Keep all counters in dining room clean and wiped down.

  • Refill condiments as needed.

  • When fresh flowers are used as center pieces, change out as directed by supervisor.

  • Prepare nourishment and snacks as requested.

  • Assist with food preparation as directed by supervisor.

  • Assist in serving meals and tray preparation for residents as required.

  • Clean dining room, tables, chairs, floors, etc. Vacuum floors as needed and after each meal.

  • Clean food preparation area, including utensils used and equipment; wash dishes, take out trash, mop floors.

  • Responsible for assisting with the daily cleaning and weekly deep cleaning of the kitchen dish room, pantry and dining room.

  • Follow safety regulations and precautions at all times.

  • Follow established Infection Control and Universal Precautions policies and procedures when performing daily tasks.

  • Report all hazardous conditions/equipment to your supervisor immediately.

  • Report all accidents/incidents to your supervisor on the shift in which they occur.

  • Dispose of food and waste in accordance with established policies.

  • Carry out special duties or projects as assigned.

  • Maintain confidentiality of all pertinent resident care information.

  • Knock before entering a resident's room when delivering or picking up trays.

  • Report complains to the Food Service Director.

  • Adhere to the facility dress code.

  • Adhere to the facility attendance policy.

  • Provide consistent customer service to residents, guests and staff.

Physical Demands:

This position is classified as HEAVY, requiring exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently and/or 10 to 20 pounds of force constantly to lift/move objects. Functional requirements including constant standing and walking. Frequent lifting, carrying, pushing, pulling, stooping, bending, turning and stretching. Finger and hand dexterity required. Visual and hearing acuity along with the ability to understand and carry out written and oral instructions are essential.

Additional Details * Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team. * Makes the initial and|or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within State specific guidelines, and submits accurate, complete, and timely documentation, per policy. * Regularly evaluates and re-evaluates (as warranted by changes in condition but at least every 60 days) the patient's nursing needs. * Performs patient comprehensive assessments at designated time points and develops the appropriate POC, in collaboration with physician orders. * Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source. * Initiates, develops, implements and makes necessary revisions to the plan of care in collaboration with the physician and other health care professional's involved in care. * Makes referrals to other disciplines, as indicated by patient's assessed need. * Responds to outcome coordinator|coder and Patient Care Manager requests for clarification to OASIS assessments on the same day that the request for more information is sent. * Plots patient encounters for the episode and determines needed RN encounters based on patient's needs and regulations. * Instructs and supervises the patient's family|caregiver in the care of the patient and maintenance of a healthy environment forthe patient. * Actively participates in weekly case conferences. * Maintains a current and accurate patient medication profile. * After start of care (SOC) assessment, reports the status of the patient, assessed needs, and plan of care overview to the teamleader on same day (or by next business day if after hours). * Observes, records and reports to the physician and/or team leader the patient's signs and symptoms, response to treatmentand changes in the patient's condition, as appropriate. Ensures adequate Team Leader (TL) communication when physicianfollow-up is needed. * Communicates changes in visit assignments, dates of scheduled visits, and schedule changes to scheduler and Patient CareManger to ensure patient needs are met. * Communicates timely and effectively with agency personnel and ordering physician as required to process orders and OASISdata sets, schedule home visits, and deliver services to patient as ordered by physician and in accordance with applicable lawsand regulation. * Facilitates hand-off communication to RN and PCM who will cover patients in their absence, prior to scheduled days off. * Performs regular and supervisory visits according to the plan of care and submits complete visit notes within 24 hours ofcompletion visit. * Directly and/or indirectly supervises care provided by the home health aides and licensed practical vocational nurses, providesinstruction as appropriate, and assigns tasks according to State and federal regulations. * Participates in on-call rotation. * Adheres to and participates in the agency's Episode Management process. * Assists in the orientation of new agency personnel as assigned. * Completes LHC required learning courses, additional assignments per DON request, as well as any state specific requiredtraining per state regulation|practice act requirements. * Participates in the performance improvement plan and process to ensure positive patient outcomes. * All other duties as assigned. Qualifications License Requirements * Current RN licensure in state of practice. * Current CPR certification required. * Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation. Additional State Requirements * CA - One year prior professional nursing experience. * LA: At a minimum, one year of clinical experience as a Registered Nurse. One year of clinical experience as a Registered Nursemay be waived for a Registered Nurse with recent clinical experience as an LPN.
