The Peritoneal Dialysis (PD) & Home Hemo Registered Nurse (RN) educates and cares for our home hemodialysis and peritoneal dialysis patients, following up with them on a regular basis. The Dialysis Home Hemo RN also assists in the ongoing development of the local home training program.

This position is part-time, 20-29 hours/week.

Responsibilities

  • Assists in the examination and evaluation of patient candidates so that appropriate PD treatment and other services can be arranged.

  • Train patients and family who have chosen peritoneal dialysis as their treatment modality.

  • Provides counseling and emotional support.

  • Check dialysis equipment and ensure it is operating properly, make adjustments, or have them made by maintenance personnel.

  • Teach the patients proper care between dialysis to avoid infections, and instruct patients on matters of diet.

  • Keep comprehensive records of the schedule and type of treatment and changes in the condition of the patient.

  • Check in with the patient at least monthly by phone or during clinic visits.

  • Obtain routine laboratory work to include routine adequacy testing.

  • Make surveillance of the patient's home adaption, including provisions for visits to the home.

  • Ensure the patient is provided consultation with a Social Worker and Dietitian.

Qualifications

  • A Registered Nurse with a current license for the applicable state

  • At least 12 months clinical experience.

  • Must have at least 3 months experience in Peritoneal Dialysis Nursing.

VIEW JOBS 1/14/2021 12:00:00 AM 2021-04-14T00:00 This is a full time (0.8 - 1.0 Status), day shift RN Nurse Navigator position in the Palliative Care Clinic. This position acts as an advocate for patients who are living with serious chronic illnesses and those nearing the end of their lives and is responsible for the coordination of care and services for the identified patient populations over an extended period of time across inpatient, outpatient and community settings. This position conducts comprehensive appraisals of patients' spiritual, psychosocial and physical needs and works closely with the Palliative Care Nurse Practitioner and other members of the multidisciplinary team to achieve optimal clinical outcomes. Minimum Required: Must be licensed as an RN. Must have a minimum of two years experience in palliative care, hospice or home care nursing and/or navigation role. Must have excellent communication, both written and verbal, and interpersonal skills. Includes the ability to educate physicians, staff and patients in the arena of palliative care and end of life issues. Must have current computer skills and ability to navigate several IT systems. Preferred/Desired: BSN preferable. Desire five years of nursing experience in area of navigation. Certification in palliative care nursing specialty preferred.
