The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant's years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

What You Will Do

The Diabetes Specialist Educator, assesses the health and educational needs of patients, using specialized knowledge and skills relating to diabetes and related conditions. The Diabetes Specialist Educator can serve as a clinical expert, educator and consultant in the outpatient and/or inpatient settings. The Diabetes Specialist Educator provides clinical expertise and support to related health care personnel and other entities regarding program development and departmental operations.

Required Qualifications

  • Bachelor's Degree Dietetics/Nutrition

  • 3 Years Experience as a Clinical Dietitian

  • Certified Diabetes Care and Education Specialist (CDCES) - Certification Board for Diabetes Care and Education

  • REQUIRED
  • CDR Registered Dietitian (RD) - Commission on Dietetic Registration (CDR)
  • REQUIRED

  • Certified Diabetes Care and Education Specialist (CDCES) - Certification Board for Diabetes Care and Education must be obtained with 90 days of hire. Registration with the Commission on Dietetic Registration

Essential Functions

  • Clinical Expert/Consultant

Collaborates with the diabetes team to update clinical guidelines, procedures and policies.

Maintains unit identified competencies, incorporating age-specific criteria as appropriate.

Develops mutually collaborative, professional relationships with other departments/entities/external health care agencies.

Collaborates with interdisciplinary team to integrate research-based interventions that enhance patient outcomes.

Acts as a resource on complex patient care activities including persons with type 1 diabetes, chronic kidney disease, organ transplant, insulin pumps, continuous blood glucose monitors and complicated pregnancies.

Obtains, reviews and analyzes information in collaboration with the patient, family, significant other, healthcare team members, employers, legal representative, and claims/insurance personnel as indicated.

Effectively schedule outpatients for assessment/evaluation and intervention with Diabetes Team.

Completes a comprehensive patient assessment that includes all aspects of Diabetes (DSME) from American Diabetes Association (ADA) guidelines and/or Medical Nutrition Therapy (MNT) guidelines from the Academy of Nutrition and Dietetics (AND).

Develops a comprehensive plan of care to achieve desired outcomes, and documents appropriately in EMR.

Coordinates and evaluates the patients' and family's use of resources and services in a quality-conscious, cost-effective manner. Anticipates potential patient outcomes and takes appropriate action when care deviates or is not progressing.

Analyzes patterns of variance from the standardized Clinical Pathways and implements strategies to resolve them.

Participates in Sharp HealthCare quality assurance programs at all levels, with the development, monitoring and evaluating of set program goals and objectives.

If applicable, manage and educate patients with diabetes in pregnancy/gestational diabetes through ongoing assessment, treatment planning and communication with OB/GYN physicians regarding plan of care for the duration of pregnancy.

If applicable, manage and educate patients with diabetes with renal and/or cardiac disease through ongoing assessment, treatment planning and communication with physician specialists (i.e, Cardiologist, Nephrologist) caring for the patient.

Provides community education services as necessary.

Participates in community organizations as representative of hospital.

Assists in the development of programs specific to diabetes that network with other Sharp Healthcare programs and community organizations.

  • Educator

Assesses the patient's clinical and psychosocial status, diagnosis, and treatment plan to ensure appropriateness.

Obtains, reviews and analyzes information in collaboration with the patient, family, significant other as related to educational needs.

In collaboration with health team members, implements a comprehensive multidisciplinary training education plan indicated to achieve targeted clinical outcomes associated with diabetes and its comorbidities.

Sets patient-centered education goals in collaboration with healthcare team and implements plan modifications when necessary.

Identifies patient risk factors derived from the patient assessment data and refers to appropriate health intervention programs when care deviates or is not progressing.

Applies effective teaching methods appropriate to the patient's age, developmental level, gender, cultural background, language and education.

Abides by the ADA DSME Standards as evidence by organization passing annual recognition requirements Skills Roster. Performs skills according to Department Based Skills Roster.

Incorporate behavioral learning styles into group presentations.

Present to multiple levels of learners simultaneously. Applies effective teaching methods appropriate to the patient's age, developmental level, gender, cultural background, language and education.

Facilitates group discussions to aid in the learning process.

Identifies patient risk factors derived from the patient assessment data and refers to appropriate health intervention programs when care deviates or is not progressing.

  • Financial Responsibilities

Collaborates with the financial department regarding appropriate billing guidelines and practices.

Obtains appropriate registration and billing information (i.e, COA, copy of insurance card). Updates financial information as necessary with financial department.

Where indicated, documents appropriately for DSME and MNT services for billing purposes.

Variable expense per unit of service does not exceed budgeted expense unit of service. Demonstrates/contributes cost-reducing or revenue-enhancing idea/actions.

In conjunction with leadership, develops fiscal year capital requirements plan.

  • Professional Competency

Maintains specialized skills certifications.

Maintains CEUs according to most current Association of Diabetes Care and Education Specialists (ADCES), ADA, and AND requirements.

Thorough knowledge of the latest diabetes medical care and management in regards to outpatient/inpatient care delivery programs.

Thorough knowledge of diabetes technology including but not limited to insulin pumps, continuous blood glucose monitors and glucometers.

Thorough knowledge of legal, ethical, professional and organizational principles and standards.

Knowledge of preventive medicine, interdisciplinary team approach and outpatient care delivery programs.

  • Performance Improvement

Participates in all service line activities including quality monitoring.

Responsible for actions, which result in improved department outcomes.

Knowledge, Skills, and Abilities

  • Advanced clinical skills in area of expertise.

  • Thorough knowledge of diabetes care, and current standards of practice.

  • Effective interpersonal skills: oral, written and presentation.
