The primary focus of this position is to provide diabetes education to adult patients at the Joslin Diabetes Center, Upstate University Hospital (UUH) or Community General Hospital (CGH) and ensure the quality standards of the Joslin Diabetes Center and throughout the Upstate campuses are met. This position will interact with staff members and/or providers in a consultative fashion to assist in the provision of diabetes education. Major duties and responsibilities include: providing education in diabetes self management to patients and their families newly diagnosed with diabetes and those patients managing their diabetes for years; developing and implementing an individualized education plan based on a needs assessment; working closely with the multi-disciplinary UUH and CGH staff as well as the Boston Joslin Diabetes Center to develop policies and protocols pertaining to diabetes; manage triage and calls as assigned for diabetes care and management; play an active role in the American Diabetes Association (ADA) Advisory Council and Recognition activities; participate in ongoing staff education and community services events; participate in the development of diabetes curricula and policies/procedures related to diabetes management; maintain accurate records in the electronic medical record and maintain the necessary professional continuing education units required.
RN with NYS license, Bachelor's of Science in Nursing, health or education related field and 3 years of clinical experience (preferably with diabetes care and management) OR RN with NYS license, Associate's degree in Nursing and 5 years of clinical experience required. Certified Diabetes Educator (CDE) or ability to obtain CDE within 4 years of hire required.
Excellent written/oral communication skills and strong computer skills required. Master's degree preferred. CPR-BLS certification required by time of appointment.
