This outpatient clinic is located in sunny San Diego, CA and minutes from our beautiful beaches and central to the vibrant La Jolla community. Serves patients with diabetes, and those at risk for diabetes and other chronic disease. Performs outreach, disseminates information, and provides self-management support.
Participates in a variety of community events and health fairs. Supports an integrated, multidisciplinary team based approach to patient care, families and communities. Provides education on a variety of health topics with a focus on prevention of chronic disease and self-management skills.
Provides assistance with access to health services as appropriate. Responsible for maintaining a list of community contacts and referrals. Assists with development, review, and revision of policies, procedures, protocols, curriculum and materials.
Supports diabetes coalition building to provide more access and resources for people with diabetes. Supports advocacy for people with diabetes. Operates within professional lines of responsibility and communication.
Please note this position has a varied work schedule and the hours are listed as 8AM to 5PM.
Ability to interact effectively with diverse groups.
Demonstrated experience and ability to work independently and within a team based structure.
Ability to set priorities and goals to ensure program objectives are met with minimal direction.
Ability to handle problems, propose solutions, and implement them.
Experience in teaching or instructing individuals and groups.
Experience using Microsoft office.
BS for Health Care Provider via American Heart Association.
One of the following: Certified Community Health Worker, Medical Assistant, Dietetic Technician, Registered Pharmacy Technician, or Licensed Vocational Nurse, Diabetes Prevention Program Lifestyle coach certification.
Experience teaching, health coaching or promoting community health.
