The Outpatient Diabetes Clinician is responsible for outpatient program implementation and other assigned program aspects that address objectives specific to diabetes education. The Inpatient Diabetes Clinician is responsible for inpatient program implementation and other assigned program aspects that address objectives specific to diabetes education. # ESSENTIAL FUNCTIONS AND RESPONSIBILITIES (50%) # Job Duties include the following, other duties may be assigned. Both Outpatient and Inpatient Provides medically necessary diabetes self-management training to patients per consult/referral or identified need.# Ensures that all components of the program meet the Quality Assurance Standards as established by DHS, ADA, and other credentialing bodies, i.e. HFAP. Contributes to the quality improvement, performance improvement, and financial stability of the department and /or organization by:# participating on a quality improvement team,# identifying improvement strategies for the department/organization, training new staff, working to meet department or ON-TARGET goals, or working on cost-savings measures. Modifies or develops practice guidelines, standards of care, or policies and procedures related to diabetes Schedules hours and supplemental activities to serve customer#s needs Maintains knowledge as measured by department competency program Serves as a primary resource to health system staff to meet overall diabetes education needs Documents all patient interactions or recommendations in the electronic health record (EHR) per hospital standards Ensures training and patient education materials are up to date with an annual review process Participates in determining and measuring annual metrics for the quality improvements for the department # Outpatient Specific (Level 1 or 2 Clinician as defined by AADE/ADA Competency Standards) Participates in the marketing and media plans established for the Diabetes Center.# Based upon outpatient program objectives, determines appropriate metrics to measure, analyze, and interpret results for clinicians and administrative staff. Establishes Diabetes Center-specific outcome measures for aggregate clinical diabetes populations, i.e. elderly, those with chronic complications, pregnancy. Participates in the establishment of the ADA recognition application process and ensures on-going compliance with these standards


Inpatient Specific Completes all consults at the earliest possible time based upon patient needs Uses EHR data to prioritize patient needs to:


Prepare the patient for self-management after discharge as appropriate Provide recommendations to improve care for patients with targeted abnormal lab values Work with other hospital departments to ensure HFAP requirements related to diabetes care are met for inpatients Provides educational sessions for new hospital staff as appropriate for their role Participates in other hospital team meetings/committees as necessary to ensure hospital-wide continuity of care. # # JOB Requirements # QUALIFICATIONS############## ############################## # Qualifications to perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The Diabetes Clinicians are required to:


Obtain the CDE (Certified Diabetes Educator) certification within 2 years of working in the department Obtain the Indiana Diabetes Educator License within 6 months of becoming eligible per requirement in the law Once CDE Certification is obtained, maintain ongoing certification

SUMMARY OF THE JOB
