Diabetes Care Specialist


The primary focus of this role is to provide education


, training, and support to patients who are new to insulin therapy, correction techniques, as well as training on Continuous Glucose Monitors (CGMs) and insulin pumps. The ideal candidate will have a strong background in diabetes management, patient education, and clinical expertise to empower patients to manage their condition effectively. The Diabetes Care Specialist is responsible for performing care management for patients with complex and chronic care needs by developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize patient health care across the care continuum and ensuring patient access to services appropriate to their health needs. Job Responsibilities Educate patients and their families about diabetes management, insulin therapy, and glucose monitoring techniques. Conduct training sessions on CGM devices, including sensor insertion, calibration, and data interpretation. Analyze CGM reports to identify patterns, trends, and areas for improvement in diabetes management Provide comprehensive training on insulin pump therapy, including pump operation, infusion set changes, and troubleshooting. Review and interpret pump data downloads to optimize insulin delivery and glycemic control. Collaborate with physicians, nurse practitioners, dietitians, and other healthcare professionals to coordinate comprehensive diabetes care. Provides targeted, proactive, relationship-based (longitudinal) care management to Diabetic patients identified as at increased risk, based on a defined risk stratification process and patients who are likely to benefit from care management. Supports patients in an effort to make them successful while following their written plan of care. Encourages patient to use self-management tools as provided. Provides information to patients and families regarding community resources, medication assistance, and other healthcare needs. Collaborates with diabetes education program on the delivery of program content and services, including tracking patients using technologies in their care to assist with DM management. Collaborates with office supervisor on improvements to workflow and effectiveness of care. Assists in the coordination of transitions of care of hospitalized patients with diabetes. -??? Skills, Experiences # Educational Requirements State-licensed Registered Nurse (RN) Certified Diabetes Educator (CDE) preferred but not required. Minimum of 1 year of clinical experience in diabetes management, endocrinology, or related specialty. Proficiency in using CGM systems, insulin pumps, and diabetes management software preferred. Strong communication, teaching, and interpersonal skills. Ability to work independently, prioritize tasks, and adapt to changing patient needs. Commitment to continuous learning, professional development, and evidence-based practice in diabetes care. Demonstrate proficiency in computer skills including Microsoft Word, Outlook, and Excel, with the ability to quickly adapt to new software applications as required. Prior experience and familiarity with electronic health records (EHR) systems required. Must have strong organizational skills and attention to detail. Strong research and analysis skills highly preferred. Must successfully function in a fast-paced, service-oriented environment. Diabetes Care Specialist
